Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the contractor was effective.
Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.

Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

Performance was not observed or not applicable to the current effort being reported against.

Instructions:

Based on your knowledge of the contract identified above, please provide your assessment of how well the contractor performed on each of the following topics. Only performance within the past five (5) years, whether completed or current, is relevant.

Circle the appropriate rating (letter) for each item on the questionnaire and please provide illustrating strengths and weaknesses that support your rating. This questionnaire will be used to evaluate contractor's Present/Past Performance to evaluate for awarding a specialized acquisition security support contract. Please answer the following questions in the context of the contractor's ability to provide support to all security disciplines required for execution of a special access program security support.

Section 1: Personnel Qualifications and Performance 1.1) How well did the contractor perform in providing qualified personnel with appropriate clearance/accesses? Please include remarks that provide detail as to any challenges encountered or laudatory performance with respect to Contractor security clearances.

1.2) How well was the contractor able to retain their cleared, qualified, and trained personnel? Were they able to maintain a fully-staffed workforce, and complete timely hiring actions when personnel departed or new positions were created? Please include remarks that provide detail as to any challenges encountered or laudatory performance with respect to hiring and retention.

E V S M U N 1.3) How well did the contractor perform in providing "turn key" personnel which required little or no training? "Turn key" is to be fully capable of performing PWS requirements on day one of performance (e.g. Possessing all required certifications, familiarity and past experience with Government programs/processes/requirements, etc.). Please include remarks that provide detail as to any challenges encountered or laudatory performance with respect to overall levels of KTR “turn key” performance readiness.

E V S M U N 1.4) How well did the personnel perform regarding Information Security, Physical Security , Personnel Security , Security Education & Training, Cybersecurity, Security Support to Operational Units, and Acquisition and Program Security Support. As instructed in the instructions, please give examples of good and bad performance. Use a continuation sheet if necessary to elaborate.

Section 2: Acquisition and Industrial Security

2.1) Did the contract involve the development of Program Protection Plans (PPP)? If so, please provide remarks that detail how well the contractor performed in development and delivery of PPPs.

2.2) Did the contract involve direct industrial security oversight or assistance with contractor locations? If so please provide remarks that detail how well the contractor performed in reviewing other contractor security plans (i.e. RMF Package, FFC, SOP, and OPSEC Plans)? What was the quality of the comments? Were the comments 100% correct IAW the security oversight requirements?

E V S M U N 2.3) Did contractor personnel have a clear knowledge of SCI and SAP security-related ICDs, JSIG, DoDMs, and legacy JAFAN/DCIDs?

E V S M U N 2.4) Did contractor personnel have the skill and knowledge to advise the Government or to conduct industry security reviews in support of or, as representatives of the Government? If they conducted such reviews, were their Findings/Deviations consistent with the ICD, DoDM, JSIG, or legacy DCID/JAFAN requirements?

2.5) Overall, how well did the contractor perform in managing and operating your Document Control Center? Did you receive anything lower than a Satisfactory when subjected to a security review by the cognizant security authority? If possible please provide comment to elaborate.

E V S M U N Section 3: Cybersecurity 3.1) Did the contractor perform cybersecurity work related to the oversight of your industrial base? Did they perform JSIG/RMF (or legacy DIACAP or DCID 6/3) reviews? If so, how well did they perform? Please provide comment to elaborate.

3.2) Did the contractor perform cybersecurity work related to the assessment and accreditation of mission systems? Did their personnel have sufficient skill and knowledge to correctly advise government Program Managers as to the cybersecurity aspects of their Programs? If so, how well did they perform? Please provide comment to elaborate. Please distinguish if the work was performed under legacy DIACAP, DCID 6/3, or current RMF/JSIG regulations.
