The Air Force Nuclear Weapons Center’s Intercontinental Ballistic Missile
Systems Division (ICBMSD) of Air Force Materiel Command is in the process of selecting a contractor for the Integration Support Contract (ISC). The ISC will provide knowledge based sustaining engineering support to the ICBMSD for sustainment of the
ICBM weapon system. The current operational ICBM, Minuteman III (MMIII), is expected to remain in the AF inventory until 2030. This poses unique challenges in sustaining mission capability for an aging weapon system well beyond its original design life. All effort under this contract will fall into two primary requirement areas: sustaining engineering and modification support. The ISC vehicle will be used by the ICBMSD and
Global Strike Command to address sustainment and modification development issues for the ICBM weapon system.
One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort. The Government depends on information received from agencies such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on prior contracts.
The areas of interest in the offeror are summarized in the enclosed questionnaire.
The schedule is extremely tight, and the Government needs your written response no later than 14 calendar days after your receipt of this letter. This schedule will allow the
Government sufficient time to analyze the data prior to the start of negotiations.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically or by hand and emailed to Mr. Kirk Dean at
Kirk.Dean@hill.af.mil and David Lovett at David.Lovett@hill.af.mil. Your completed questionnaire will become a part of the official Source Selection records.

SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE
RELEASE OF PAST PERFORMANCE INFORMATION TO THE PRIME
CONTRACTOR
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent.
Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past performance information to the prime during exchanges. In an effort to assist the Government in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Past Performance Volume
(or Proposal).

COMMERCIAL CLIENT AUTHORIZATION LETTER
To assist the Government in performing a Performance Confidence Assessment Group of your present and past performance on relevant commercial contracts, the following letter shall be issued to your points of contact (POCs) for those commercial efforts that you identify to us in your past performance volume. A copy of the authorization letters for each commercial contract shall be included in your Past Performance Volume.
