SAMPLE QUESTIONNAIRE COVER LETTER

The Aerospace Dominance Enabler Division of Air Force Materiel Command is in the process of selecting a contractor for the Advanced Radar Threat System-Variant 2 (ARTS-V2). ARTS-V2 is a surface-to-air missile (SAM) threat radar based on a foreign fielded, medium range, “double digit”, SAM threat system. The ARTS-V2 is designed to be used at Department of Defense (DoD) training ranges for aircrew training and tactics development to increase combat effectiveness and aircrew survivability by training aircrews to engage or defend against an advanced SAM threat before encountering it in actual combat. Various aircraft platforms will train against the ARTS-V2, but the most stringent requirements placed on the ARTS-V2 system design come from 5th Generation aircraft capabilities.
One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort. The Government depends on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on prior contracts.
The areas of interest regarding the offeror are summarized in the enclosed questionnaire.
Our schedule is extremely tight, and the Government needs your written response no later than 14 calendar days after your receipt of this letter. This schedule will allow the Government sufficient time to analyze the data prior to the start of negotiations.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically or by hand and emailed to Mr. Cory Hansen at cory.hansen@us.af.mil. Your completed questionnaire will become a part of the official source selection records.
Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this source selection.
