SOURCE SELECTION
1. Program Structure and Objectives
1.1. The Government intends to select up to seven small business contractors for the Multiple Award
Construction Contract (MACC) II program. However, the Government reserves the right to award more or fewer total contracts if the Source Selection Authority (SSA) determines it is in the Government's best interest. The SSA will consider the Government's expected volume of work, available funding, adequate competition, and the availability of best value proposals. The Government reserves the right to award contracts without discussions. Each contract will have a one year ordering period with four-one year options. Individual task orders will be competed under fair opportunity rules cited in Federal Acquisition
Regulation (FAR) 16.
1.2. Budget/Funding Information - Funding will be obligated on each task order awarded under the basic contracts.
2. General Instructions
2.1. This section provides general guidance for preparing proposals as well as specific instructions on the format and content of the proposal. The Offeror's proposal must include all data and information requested and must be submitted in accordance with these instructions. Any Offeror who submits an incomplete package may be considered non-responsive and eliminated from the competitive range. The
Offer shall be compliant with the requirements as stated in the Statement of Work (SOW), Labor
Categories, and SF 1442 Solicitation/Contract. Non-conformance with these instructions may result in an unfavorable proposal evaluation.
2.2. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal shall not simply rephrase or restate the
Government's requirements, but provide convincing rationale to address how the Offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of their facilities and experience, and will base its evaluation on the information presented in the Offeror's proposal.
2.3. Elaborate brochures or documentation, binding, detailed artwork, or other embellishments are unnecessary and are not desired.
2.4. The proposal acceptance period is specified in Block 13 (D) of Section A (front page) of the model contract/solicitation. Offerors providing less than the specified calendar days for Government acceptance after the date offers are due may be considered non-responsive and may be eliminated.
2.5. In accordance with FAR Subpart 4.8 (Government Contract Files), the Government will retain one copy of all unsuccessful proposals. The Government will destroy extra copies of such unsuccessful proposals unless the offeror requests otherwise.
2.6. Point of Contact - The Contracting Officer (CO) is the sole point of contact for this acquisition. Address any questions or concerns you may have to the CO. Written requests for clarification may be sent to the

2.7. Pre Award Debriefings - The CO will promptly notify offerors of any decision to exclude them from the competitive range, whereupon they may request and receive a debriefing in accordance with FAR 15.505.
2.8. Post Award Debriefings - The CO will notify unsuccessful offerors in the competitive range of the source selection decision in accordance with FAR 15.506. Upon such notification, unsuccessful offerors may request and receive a debriefing. Offerors desiring debriefing must make their request in accordance with the requirements of FAR 15.505 or 15.506, as applicable.
2.9. Discrepancies - If an Offeror believes that the requirements in these instructions contain an error, omission, or are otherwise unsound, the Offeror shall immediately notify the CO in writing with supporting rationale. The Offeror is reminded that the Government reserves the right to award this effort based on the initial proposal, as received, without discussion.
2.10. Electronic Reference Documents - All referenced documents for this solicitation are available on the
Federal Business Opportunities (FedBizOpps) website at http://www.fedbizopps.gov. Potential Offerors are encouraged to subscribe for real-time e-mail notifications when information has been posted to the website for this solicitation, including updates.
2.11. Oral Presentation - Oral presentations will not be allowed.
2.12. Amendments to Solicitation - If this Request for Proposal (RFP) is amended, all terms and conditions that are not amended remain unchanged. Offerors shall acknowledge receipt of any amendment to this request by the date and time specified in the amendment(s) by signing and returning the front page of each.
2.13. Submission, Modification, Revision, and Withdrawal of Proposals
2.13.1 Proposals and modifications to proposals shall be submitted in both paper and electronic media to the address located in paragraph 2.6 of this document and shall include the time and date specified for receipt, the solicitation number, and the name and address of the Offeror. Late proposals will only be considered IAW FAR 15.208.
2.13.2 Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the proposal wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel.
2.13.3 Proposals may be withdrawn by written notice received at any time before award. Proposals may also be withdrawn in person by an Offeror or an authorized representative, if the identity of the person requesting the withdrawal is established and the person signs a receipt for the proposal before award.

2.14 Organization/Number of Copies/Page Limits
2.14.1 The title page of each volume must show-

2.14.2 The Offeror shall prepare the proposal as set forth in the Proposal Organization table below. The titles and contents of the volumes shall be as defined in this table, all of which shall be within the required page limits and with the number of copies specified. The volumes identified in the table shall be separately bound in three-ring, loose-leaf binders. The contents of each proposal volume are described in the ITO paragraph as noted in the table below.

2.14.3 Page Limitations - Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal and (for paper copies) will be shredded. Page limitations may be placed on responses to Evaluation Notices (ENs). The specified page limits for EN responses will be identified in the letters forwarding the ENs to the
Offerors. Each page shall be counted except the following: blank pages, title pages, tables of contents, tabs, glossaries, Cross Reference Matrix, and those noted in the Proposed Organization
Tables and Figures as “Unlimited.”
2.15 Pricing Information - All pricing information shall be addressed ONLY in the Price
Proposal.
2.16 Cross Referencing
2.16.1 Each volume shall be written on a standalone basis so that its contents may be evaluated without cross-referencing to other volumes of the proposal. Information required for proposal evaluation that is not found in its designated volume will be assumed to have been omitted from the proposal.
Cross-referencing within a proposal volume is permitted.
2.16.2 The Offeror shall fill out a cross reference matrix in the format below, indicating the proposal reference information as it relates to the Section L - Instructions to Offerors, and Section M -
Evaluation Factors for Award. This shall be included in the Technical Volume (Volume II). The cross reference matrix addresses each requirement within the SOW and subfactors. The Offeror shall fill in each section independently labeled with the appropriate paragraph number or page number from their proposal that addresses the requirement or subfactor.

2.17 Indexing
2.17.1 Each volume shall contain a detailed table of contents to delineate the subparagraphs within that volume. Tab indexing shall be used to identify sections.
2.17.2 Include an indexed list of Tables and Figures. Tables and Figures do not count against the page limitations for their respective volumes but should be used only as necessary and kept to a minimum.
2.17.3 Glossary of Abbreviations and Acronyms - Each volume shall contain a glossary of all abbreviations and acronyms used, with an explanation for each. Glossaries do not count against the page limitations for their respective volumes.
2.18 Page Size and Format
2.18.1 Page size shall be 8.5 x 11 inches, not including foldouts, and printed on both sides. Sheets shall be double-sided which will count as two pages. Except for the reproduced sections of the solicitation document, the text shall be Arial, font size shall be no less than 11 points, and lines shall be double spaced. Tracking, kerning, and leading values shall not be changed from the default values of the word processing or page layout software. Use at least 1 inch margins on the top, bottom, and sides. Pages shall be numbered sequentially by volume. These page format restrictions shall also apply to responses to ENs. These limitations shall apply to both electronic and hard copy proposals.
2.18.2 Legible tables, charts, graphs and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc. These displays shall be uncomplicated, legible and shall not exceed 11 x 17 inches in size. Foldout pages shall fold entirely within the volume, be printed only on one side, and count as a single page. Foldout pages may only be used for large tables, charts, graphs, diagrams and schematics; not for pages of text. For tables, charts, graphs and figures, the text shall be no smaller than 10 point.
2.19 Binding and Labeling - Each volume of the proposal should be provided separately in a three-ring loose-leaf binder. Staples shall not be used. Be sure to apply all appropriate markings including those prescribed in accordance with FAR 52.215-1(e), Restriction on Disclosure and Use of Data, and 3.104-4, Disclosure, Protection, and Marking of Contractor Bid or Proposal Information and Source Selection
Information.
2.20 Electronic Offers - The content and page size of electronic copies must be identical to the hard copies. In the event there is a discrepancy between the hard copy and electronic copy, the hard copy shall take precedence. For electronic copies, indicate on each CD the volume number and title. Each volume shall be on a separate CD. Use separate files within each CD to permit rapid location of all portions, including subfactor required plans, exhibits, annexes, and attachments, if any. The Offeror shall submit Volumes I and II in electronic format, using read only CDs (i.e. CD-R). Re-writable CDs (i.e. CD-RW) shall NOT be used. If files are compressed, the necessary decompression program must be included. The electronic copies of the proposal shall be submitted in a format readable by Microsoft (MS) Word 2007, MS Excel
2007, MS-Project 2007and MS-Power Point 2007, as applicable. PDF formats shall ONLY be used for drawings, cut sheets, and related type documents.
2.21 Distribution - The "original" proposal shall be identified. Proposals shall be addressed to the Contracting
Officer and mailed or hand-delivered to:

3 Volume I - Price
3.13 General Instructions
3.13.1 The primary purpose of the Price Proposal Volume is to provide the CO and the team evaluators an understanding of how the price proposal was developed. The price evaluation criteria of completeness, reasonableness, and Total Evaluated Price (TEP) as defined in Section M of the
RFP will be assessed from the Offeror’s information provided in this volume.
3.13.2 Submission - Submit the price proposal (Contract Cost Estimate worksheet, Pricing Assumptions, Estimating Techniques, etc.) in Microsoft Word and Microsoft Excel worksheets as appropriate.
Word documents and Excel spreadsheets must be editable and not password protected. Each disk shall be clearly labeled. The paper and electronic copies shall be clearly identified. Scanned documents in PDF files are NOT acceptable for the Price Proposal Volume.
3.13.3 Total Evaluated Price (TEP): For pricing, the Government will evaluate Seed Project proposals based on a TEP. Offerors shall input prices for all categories identified in the Contract Cost
Estimate worksheet (Section J attachment). The TEP is Firm Fixed Price, contractually binding and shall include all costs associated with completing the Seed Project.
3.13.4 Cost or Pricing Information Requirements - In accordance with FAR 15.402, 15.403-1(b) and
15.403-3(a), “information other than cost or pricing data” may be required to support price reasonableness. If, after receipt of proposals, the CO determines that there is insufficient information available to determine the reasonableness of the proposed pricing the Offeror shall be required to submit additional information other than cost or pricing data. If, during the source selection process, the CO determines that adequate competition no longer exists, then cost and pricing data may be required.
3.13.5 Pricing Assumptions - Summarize all significant pricing assumptions, scope limitations and/or qualifications of the Price proposal. The Offerors’ proposal shall be valid through time of award.
3.13.6 Estimating Techniques and Methods - When responding to the Price Volume requirements in the solicitation, the Offeror and associated subcontractors shall use generally accepted construction estimating techniques, such as RS Means or equivalent to develop their estimates on the Seed
Project.
3.13.7 Price Information - Information beyond that required by this instruction shall not be submitted unless you consider it essential to document or support your price position. All information relating to the proposed prices/rates, including all required supporting documentation must be included in the section of the proposal designated as the Price Volume. Under no circumstances shall this information and documentation be included elsewhere in the proposal.
3.13.8 Rounding - All total dollar amounts provided shall be rounded to the nearest dollar.
4 Volume II - Technical Acceptability
4.1 General - The Technical Acceptability Volume should be specific and complete. Legibility, clarity and coherence are very important. Responses will be evaluated against the Technical Subfactors defined in
Section M. Address each subfactor listed below.
4.2 Subfactor One: Seed Project - Renovation of Building 517 (KRSM 112011)
4.2.1 Submit a basis of design for the seed project in accordance with the Seed Project SOW (Section J
Attachment). The Government reserves the right to award this project within six months of basic contract award.
4.2.2 The basis of design shall include:
4.2.2.1 A detailed analysis which includes a comprehensive set of engineering calculations for electrical (Short Circuit Ground Fault Analysis, Circuit Breaker Coordination Study, and
Voltage Drop Calculations), mechanical to include calculations for loads and equipment sizing, structural to include seismic analysis and fire protection disciplines. Preliminary calculations for electrical, mechanical, and fire protection disciplines are acceptable.
Structural calculations must be complete and stamped by a registered engineer.
4.2.2.2 A detailed explanation about how the contractor intends to comply with Air Force Sustainable
Design and Development (SDD) Implementing Guidance dated 02 June 2011.
4.2.2.3 Compliance with Base Facility Design Standard and the Base Architectural Compatibility
Standard.
4.2.2.4 Compliance with all Unified Facilities Criteria (UFC) Design and Specifications Guidance.
4.2.3 Submit a Seed Project Construction Quality Control Plan in accordance with UFGS 01 45 00.00

4.2.4 Submit a Safety Plan specific to this project in accordance with Governmental Safety
Requirements UFGS 01 35 26.
4.3 Subfactor Two: Architect-Engineer (A-E) Qualifications
Submit current Architect-Engineer (A-E) Qualifications, Standard Form (SF) 330* (Section J Attachment) and ensure that it includes:
4.3.1 Evidence that the A-E team consists of licensed individual(s) with a minimum of five years design experience in each of the following disciplines: Architectural, Civil, Structural, Electrical, and
Mechanical fields.
4.3.2 Evidence that the A-E team has at least one Leadership in Energy and Environmental Design
(LEED) Accredited Professional (AP).
4.3.3 At least three example projects similar in scope in regards to complexity, cost, and size of the Seed
Project.
4.3.4 At least one example project with LEED AP Silver Certification.
4.3.5 Evidence that contractor has an established partnership with an Architect-Engineer (A-E) design firm to provide a full range of construction and engineering services for the term of the contract.
4.3.6 *Offerors may use a size 10 font and single spacing for the SF 330, however, the form itself may not be modified (i.e. margins).
4.4 Subfactor Three: Basic Contract Occupational Safety and Health Plan
4.4.1 Submit an Occupational Safety and Health Plan in accordance with Appendix A Industrial Safety and Health Requirements, of the Statement of Work (SOW). The format of the Safety and Health
Plan should follow the Appendix A format. The Offeror's Safety and Health Plan shall include all of the minimum requirements specified in the Appendix A.
Special attention is directed to the Occupational Safety and Health Act (OSHA) Public Law 91-
596 and the resulting standards, OSHA Standard 29 CFR 1910-1926, as applicable. Hill AFB is participating in the OSHA Voluntary Protection Program (VPP). Information on the VPP can be accessed at: http://www.osha.gov/dcsp/vpp/index.html.
The accepted Safety and Health Plan shall be made a part of the resultant contract either by reference or as an attachment. Failure to submit an accepted Safety and Health Plan shall make the Offeror ineligible for award of a contract.
4.4.2 Submit Total Case Incident Rate (TCIR) and Days Away, Restricted and/or Transferred (DART) rates, or, injury and illness rates for the past three years for the North American Industrial
Classification Systems (NAICS) Code 236220. For newly formed entities or other Offerors that do not possess three years of consecutive recording data, submit this information for the time period you do possess. You may also submit other comparable data from a recognized agency, such as workers compensation, insurance rates, etc, for the same time period as well. This is data the Government must keep on file as part of participating in the OSHA VPP. After award, annual reporting of Total Case Incident Rate (TCIR) and Days Away, Restricted and/or Transferred
(DART) rates, or, injury and illness rates is required.
4.4.3 The Offeror shall also address steps to maintain and/or improve submitted rates or other comparable data as appropriate in the Occupational Safety and Health Plan.
4.4.3.1 In the event the Offeror’s TCIR and DART rates or injury and illness rates are greater than the published Bureau of Labor Statistics (BLS) national average for the NAICS
Code 236220 at time of initial proposal submission, the offeror shall submit a Get Well
Plan detailing the Offeror’s plans to improve their rates.

4.5 Subfactor Four: Basic Contract Construction Quality Control Plan
4.5.1 Submit a Construction Quality Control Plan for the basic contract in accordance with UFGS-01 45
00.00 10.
4.6 Subfactor Five: Fire Protection Engineering Plan
4.6.1 Submit a Fire Protection Engineering Plan that demonstrates the capability to obtain services as required by AFI 32-10141, Page 19, Attachment 3, IAW MACC II statement of work, Section II, as well as the following:
4.6.1.1 Technical qualifications for installing a Fire Alarm System IAW UFC 3-600-01 paragraph
5.1.
4.6.1.2 Additional qualifications for combined fire alarm/mass notification systems IAW UFC 4-021-
01, paragraphs 1-5.
4.6.1.3 Technical qualifications for the design of High Expansion Foam Systems in hangars IAW
Engineering Technical Letter 02-15, paragraph A1.6.
4.6.1.4 Engineer services to meet intelligibility criteria IAW UFC 4-021-01 paragraph 4-6.1.2.
4.7 Subfactor Six: Bonding Capability
4.7.1 Submit a prequalification letter from an A.M. Best "A" rated surety (not a bonding agent), that verifies your bonding relationship and shows a minimum aggregate bond capacity of $10 Million for this IDIQ contract. The letter should also identify a single project bond amount.
5 Volume III - Contract Documentation
5.1 Model Contract/Representations and Certifications SF 1442 - The purpose of this volume is to provide information to the Government for preparing the contract document and supporting file. The Offeror’s proposal shall include a signed copy of the Model Contract and Sections A through K.
5.1.1 Section A - Solicitation/Contract Form: Complete blocks 14, 15, 16, 19, 20, and 30 of the
SF1442. Signature by the Offeror on the SF1442 constitutes an offer, which the Government may accept.
5.1.2 Section B - Supplies or Services and Costs/Prices: Do not input pricing information in Section B of the model contract. All pricing shall be included only in the Price Volume.
5.1.3 Section I - Contract Clauses
5.1.4 Section K - Representations, Certifications, and other Statements of Offerors: Complete representations, certifications, acknowledgments and statements. Offeror must have filled out an online provision in accordance with FAR Clause 52.204-08 Annual Representations and

5.1.5 Exceptions to Solicitation Requirements - Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Failure to meet a requirement may result in an offer being ineligible for award. Each exception shall be specifically related to each paragraph and/or specific part of the solicitation to which the exception is taken. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, cost, and specific requirements of the solicitation. 5.2 Other Information Required
5.2.1 Authorized Offeror Personnel - Provide the name, title, telephone number, e-mail address of the company/division point of contact regarding decisions made with respect to your proposal and who can obligate your company contractually. Also, identify those individuals authorized to negotiate with the Government.
5.2.2 Company/Division Address, Identifying Codes, and Applicable Designations - Provide company/division’s street address, county and facility code; CAGE code; and DUNS code.
5.2.3 In accordance with FAR Clause 52.219-14 Limitations of Subcontracting at least 15 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern.
5.2.3.1 Small businesses that intend to count the performance of other small businesses in meeting the 15 percent requirement of FAR Clause 52.219-14, Limitations of Subcontracting, must include in their Contract Documentation Volume evidence of a joint venture has been approved by the Small Business Administration (SBA).
6 Volume IV - Past Performance
6.1 Offerors may submit recent, relevant contracts as defined in Section M. The government will consider this information, along with information obtained from other sources (e.g. Past Performance Information
Retrieval System (PPIRS), customer interviews, etc.).
6.1.1 Offerors are encouraged to address any performance problems/issues that the Government may identify and explain corrective action taken and current status. The Government will consider this information in determining the offerors past performance rating.
6.1.2 An offeror’s failure to address performance problems/issues within this volume does not preclude the government from using such information in determining a past performance rating.
6.2 Offerors shall submit all Past Performance information on Past Performance Information Sheets (Section J

SOURCE SELECTION
1. BASIS FOR CONTRACT AWARDS
1.1. This acquisition will utilize Lowest Price Technically Acceptable (LPTA) source selection procedures in accordance with Federal Acquisition Regulation (FAR) 15.101-2, as supplemented.
Technical tradeoffs will not be made and no additional credit will be given for exceeding acceptability.
Award will be made to the technically acceptable Offerors with the lowest evaluated prices, acceptable past performance and whose proposals conform to the solicitation requirements. The solicitation requirements include all stated terms, conditions, representations, certifications, and all other information required by
Section L of this solicitation. The Government reserves the right to award without discussions.
1.2. Number of Contracts to be Awarded: The Government intends to award up to seven contracts for the MACC II program. However, the Government reserves the right to award more or less contracts or no contract at all, depending on the quality of the proposals and prices submitted and the availability of funds.
1.3. Rejection of Unrealistic Offers:The Government may reject any proposal that is evaluated to be unrealistic in terms of program commitments, including but not limited to contract terms and conditions, or unrealistically high or low in price.
1.4. Major Proposal Revision: If a proposal receives an “Unacceptable” technical rating after initial evaluation and will require major revisions in order to meet the technical requirement, the Government reserves the right to exclude the Offeror from the competitive range at this phase of the evaluation.

2.1. Evaluation Methodology:
2.1.1. A price evaluation will be performed on all offers in accordance with Section 2.2 and proposals will be ranked from lowest to highest price based on the Total Evaluated Price (TEP).
2.1.2. Next, a technical and past performance evaluation will be performed beginning with the lowest priced Offerors. Evaluations will cease when an adequate number of technically acceptable proposals receive an acceptable past performance rating. Only those Offerors determined to be technically acceptable with an acceptable past performance rating, either initially or as a result of discussions, will be considered for award.
2.1.3. Award will be made to the lowest evaluated priced proposals meeting the acceptability standards for the non-cost factors.
2.2. Volume I - Price:
2.2.1. The pricing criteria used for evaluation are (1) Completeness; (2) Price Reasonableness; and (3)
Total Evaluated Price (TEP).
2.2.2. Completeness - The Government will review the pricing submissions for completeness and compliance with Section L of the RFP. Incomplete price submissions may NOT be evaluated and the proposal may be eliminated from competition.
2.2.3. Price Reasonableness - The Offeror’s price proposal will be evaluated using one or more of the techniques defined in FAR 15.404-1 (Proposal Analysis Techniques). Generally adequate price competition will satisfy the criteria of price reasonableness.
2.2.4. Total Evaluated Price (TEP) - Price analysis will be performed to determine the reasonableness of the Offeror’s price proposal. The TEP will be calculated as the sum of all proposed prices contained in the Contract Cost Estimate Worksheet (Section J Attachment) for the Seed Project.
The TEP is a fixed and contractually binding amount, thus all applicable division price fields must be populated. An omitted price may result in proposal elimination.
2.3. Volume II - Technical Acceptability:
2.3.1. Technical proposals will be evaluated for acceptability in accordance with the criteria set forth in this section. The Offeror’s technical proposal must convey to the Government that the Offeror is capable, possesses adequate technical expertise and experience, possesses sufficient resources, and is able to plan, organize, and use those resources in a coordinated and timely fashion to meet the needs of the Government.
2.3.2. Technical Volume will be evaluated on an acceptable/unacceptable basis and assigned one of the following ratings:
2.3.3. Subfactor One: Seed Project - Renovation of Building 517 (KRSM 112011)
The subfactor minimum is met when the Offeror’s basis of design is established as sound, feasible, and effective, conforms to all applicable requirements of the Seed Project Statement of Work, and clearly demonstrates evidence of the full capability their team possesses for executing the project with emphasis on the following elements:
2.3.3.1. Basis of design is detailed and includes:
2.3.3.1.1. A comprehensive set of preliminary engineering calculations for electrical, mechanical, and fire protection requirements.

2.3.4. Subfactor Two: Architect-Engineer (A-E) Qualifications
The Subfactor minimum is met when the Offerors submission clearly demonstrates:
2.3.4.1. The team possesses licensed individual(s) with a minimum of five years design experience in each of the disciplines of Architectural, Civil, Structural, Electrical, and
Mechanical fields.
2.3.4.2. The team possesses at least one Leadership in Energy and Environmental Design (LEED)
Accredited Professional (AP).
2.3.4.3. Three example projects similar in scope in regards to complexity, cost, and size of the
Seed Project.
2.3.4.4. At least one example project with LEED AP Silver Certification.
2.3.4.5. The contractor has an established agreement with an Architect-Engineer (A-E) design firm to provide a full range of construction and engineering services for the term of the contract.
2.3.5. Subfactor Three: Basic Contract Occupational Safety and Health Plan
2.3.5.1. The Subfactor minimum is met when the Offeror’s Occupational Safety and Health Plan is approved by the Hill AFB Safety Office and meets the minimum requirements outlined in the Appendix A.
2.3.5.2. The submitted Total Case Incident Rate (TCIR) and Days Away, Restricted and/or
Transferred (DART) rates, or, injury and illness rates should be equal to or less than the most recently published Bureau of Labor Statistics (BLS) national average for the NAICS
Code 236220.
2.3.5.3. The Occupational Safety and Health Plan is acceptable when it addresses steps to maintain and/or improve submitted rates or other comparable data as appropriate.
2.3.5.3.1. If TCIR and DART rates or injury and illness rates are greater than the most recently published BLS national average, the submitted Get Well Plan details the Offeror’s plan to improve their rates.
2.3.6. Subfactor Four: Basic Contract Construction Control Plan
The Subfactor minimum is met when the Construction Quality Control Plan for the basic contract meets requirements of UFGS 01 45 00.00 10.
2.3.7. Subfactor Five: Fire Protection Engineering Plan
2.3.7.1. The Subfactor minimum is met when the Fire Suppression plan clearly demonstrates the capability to obtain the following services IAW MACC II Statement of Work, Section II:
2.3.7.1.1. Technical qualifications for installing a Fire Alarm System IAW UFC 3-600-
01 paragraph 5.1.
2.3.7.1.2. Additional qualifications for combined fire alarm/mass notification systems
IAW UFC 4-021-01, paragraphs 1-5.
2.3.7.1.3. Technical qualifications for the design of High Expansion Foam Systems in hangars IAW Engineering Technical Letter 02-15, paragraph A1.6.

The subfactor minimum is met when the offeror provides a letter from a surety with an A.M. Best rating of “A”, stating the offeror would be bonded for a minimum aggregate amount of $10
Million if awarded a basic contract under this IDIQ.
2.4. Volume III - Contract Documentation
All sections and parts of the Solicitation and SF 1442 shall be complete and accurate.
2.5. Volume IV - Past Performance:
2.5.1. Past performance information will be gathered from offeror submissions, Government databases such as the Past Performance Information Retrieval System (PPIRS) and/or through the use of
Government and commercial customer interviews and questionnaires. Only recent and relevant past performance information will be considered.
2.5.1.1. Recency is defined as performance less than three years old. The Government will not consider performance on contracts/task orders where performance was concluded more than three years prior to this solicitation’s issue date.
2.5.1.2. Relevancy is defined as work that involves approximately the same types of services described in this RFP.
2.5.2. Past performance will be rated on an “acceptable” or “unacceptable” basis using the following ratings.

Acceptable Based on the Offeror’s performance record, the
Government has a reasonable expectation that the offeror will successfully perform the required effort, or the Offeror’s performance record is unknown. (See note below)
Unacceptable Based on the Offeror’s performance record, the
Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.
Note: In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”
3. DISCUSSIONS:
It is the Government’s intent to award without discussions; therefore, it is imperative that Offerors submit their best terms initially. However, if during the evaluation period, it is determined to be in the best interest of the
Government to hold discussions, Offeror responses to Evaluation Notices (ENs), and the Final Proposal Revisions
(FPRs) will be considered in making the source selection decision.
4. SOLICITATION REQUIREMENTS, TERMS AND CONDITIONS:
Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements as detailed in the SOW, in addition to those identified as evaluation factors or subfactors. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.
