Under the definition of Highly Relevant Rating from the second bullet point it states “Provides Services for DoD medical services (Air Force, Army, Navy, TMA) in three(3) or more states

Can you clarify that it is referring to 1 contract providing services in 3 different states? Or [can this] be 3 contracts in 3 states?

The relevancy determination will be applied to each “past performance record” (see Section M3.3.4).

Questions 177-199 do not exist. The final RFP question set started with question 200 to differentiate from those questions pertaining to the draft RFP.

In completing the Pricing Schedule, Attachment 8, Table B-1, are we allowed to demonstrate an escalation in the option years for SCA positions?
Please follow the directions in paragraph 3 and 4 regarding the escalation rate that is listed in the "Instructions" tab of Attachment 8, Table B-1, Representative Task Order and L6.1.5.2, specifically the escalation rate paragraph.

Are all RTO prices on this schedule considered ceiling prices if and when we were to provide these services in the base or option years of a contract awarded from this solicitation?
The RTO prices will be used to set fully burdened hourly rates for the location-specific labor categories listed on the RTO through 30 Jun 2012. (Section H.1b, L6.1.5.2, and M3.2.4). The RTO prices do not apply to subsequent task order pricing.

RE: L-5 Past Performance Tool: If we are the Prime completing the tool for ourselves and the subcontractor(s) will do their own, then in entering requested information, item 7 on page 138 (of the RFP) did not appear after 5 & 6 before going on to 8. So we were not prompted to complete the Business Relationship screen. Is this the correct sequence? Doesn’t the prime have to demonstrate the business relationships with other companies that constitute part of their “team,” even if those other companies will be entering their own Past Performance information?
Yes, this is the correct sequence.

The business relationships are created in paragraph 9 of L-5 for Prime and all Subcontractors who are submitting their own past performance information.

RE: L-5 –item #8 on page 138: Is the place of work the location of office(s) from which the particular contractor will operate to direct operations on this contract? Or are you asking in which states within the scope of the proposed contract will said contractor potentially provide services as part of the “team?”
The place of work refers to the place where the work was performed, not the office which managed the work.

RE: L-5- page 140 “Delivery/Task Order”- if we are referencing a large MATO contract with many task orders (and not just referencing one stand alone Task Order) even though many Task Orders comprise the overall contract, should be leave that block blank?
The instructions under "Contract Dollar Value" allow you to provide total contract value or a stand-alone task order value.

RE: L-5-“Contract Dollar Value”: If the contract is a large MATO, running for a number of years where the overall contract is ID/IQ and individual Task Orders are issued as Firm Fixed Price, and the reference supplied is for that large overall contract, do you really want the stated original ceiling value from the time of initial award, even though it really has no direct relationship with the total of all the initial awards of the individual Task Orders?

The instructions in paragraph 14 of L-5 under "Contract Dollar Value" ask for an "original" (ceiling) amount and a "current" amount.

Does the requirement for $200,000/500,000/20,000 automobile liability insurance to be maintained by the company, apply only to company vehicles used in connection with the contract, or does it extend to non-company vehicles used by the workers to drive to and from work?
Any contractor employee who works on a Government Installation must have, and maintain, the coverage limits stated in section H.8.a.2.
Reference Question [217], If yes, and if the physicians are independent contractors as is common in the locum tenens industry, some carriers will not extend vehicle coverage to the physicians since they are not employees of the company. In such cases, is it acceptable to the government that the physician carries their own insurance at the stated limits and the company verifies such coverage?

Will the Internal Medicine physician be required to do inpatient/hospitalist duties?

(There is a large difference in pay/ bill rate between outpatient coverage and inpatient coverage - almost double).

If this question refers to Section J, Attachment 4, Approved Position Descriptions, then any unique inpatient or outpatient requirement will be annotated on an addendum to the position description as part of the Task Order Proposal Request. (See Section C, paragraph 1.4.2 for the definition of an addendum).

Can you please clarify the main difference(s) in job function as to the Dentist and Dentist General Clinical?
The differences are shown in the qualifications section in each of the position descriptions.
Can you provide any average statistics for MTF openings on the dental positions and what MTF’s have been the hardest to fill?
The Government will not provide market research data.
Are the Contractors required to use the government’s vendor of choice or can they use their vendor to conduct criminal background checks?
The Government does not dictate a vendor of choice.

Please refer to Section C, paragraph 2.4.19.1 regarding the Criminal History Background Check.

Are there incumbent contractors currently under a contract that this contract will replace?

If yes, can the Government please provide the name of the incumbent or incumbent contractors?

Yes. Please refer to the bold text in Section L6.1.5.2 of the RFP.

The Government provided the names of the incumbent contractors in the representative task order.

In the example where an offeror submits 5 past performance references, should “Tab 9 – Organization Change History” be labeled f or submission as “Tab 6-Organization Change History” – or should it remain as “Tab 9” regardless of number of past performance references submitted? (likewise with Tab 10)
The instructions Table L-1, Proposal Organization reserve Tabs 6-8 for references if needed. The Tab numbers in Table L-1 for the Organizational Change Structure and Glossary of Abbreviations and Acronyms are correct.

The Government has requested that the Past Performance volume be submitted 7 days prior to the closing date. Should offerors consider that submission to be complete, or, does the Government require that offerors provide another set of copies of the volume IV with volumes I-III and V when they are submitted?

The offeror will submit only one past performance volume. The Government requested the offeror submit their past performance volume seven calendar days prior to the date set in the solicitation. However, the Government will still accept the past performance volume until the time and date set in the solicitation for receipt of proposals.

In Section L.5.1.3.1.3, page 120 of the RFP under the Human Resources Plan, it explains that we must describe our professional compensation plan. Are you looking for a detailed compensation plan on each individual labor category with costs included or just a description of our compensation plan that does not include costs?
The measure of merit is described in Section M3.1.2.1.3.

In Section L.6.1.2 – Price Information on page 122, the government states that the offeror shall not submit any information beyond what is required in the instruction. Does this mean that the government is not looking for supporting detailed pricing build up data that explains how we came to our loaded labor rates?
As stated in Section L6.1.2, “Information beyond that required by this instruction shall not be submitted, unless the offeror considers it essential to document or support a price position.”

In Section L.6.1.5.3, - Exceptions/Assumptions & Rationale (Tab 3), on page 123 of the RFP, what is the page limit? Also, is the government looking for the contractor to include additional information besides a description of each of the assumptions made in the preparation of pricing?

There is no page limit for Tab 3 in Volume III as identified in Table L-1, Proposal Organization.

As stated in Section 6.1.5.3, offerors shall provide “complete rationale for each assumption.”

L5.1.2.3.2 The offeror shall also submit sample qualifying documentation in response to Table B-2, Sample Task Order Proposal Request and Letter of Intent to Award (Section J, Attachment 9) that ensures both credentialed and non-credentialed health care workers meet the preliminary Government minimum requirements in the contract and sample task order proposal request.

Question: This can be an extensive amount of documentation. Does it count in the 50 page limit? Do you want a complete application packet for each type of provider including primary source verifications and MTF paperwork?

Page limitations are outlined in Table L-1, Proposal Organization and Section L3.4.7 The measure of merit is outlined in Section M3.1.1.3.2. The definition for preliminary Government minimum requirements is outlined in Section C, paragraph 1.4.42.

L5.1.3.1.3 The offeror shall submit a human resources management plan describing a professional compensation plan IAW FAR 52.222-46 that ensures uninterrupted services for recruitment, placement, and retention of incumbent health care workers from predecessor contracts . As per the FAR clause, it states that “As part of their proposals, offerors will submit a total compensation plan setting forth salaries and fringe benefits proposed for the professional employees who will work under the contract.”

Question [a]: Do you want this information for every position listed on Attachment 8? If so, can it be provided in an attachment that does not count in the page count?

Question [b]: Providing salary (wage) information in this section appears to be in direct conflict with L.6.1.2 which states “ All information relating to the proposed price, including all required supporting documentation, must be included in the section of the proposal designated as the Price volume. Under no circumstances shall this information and documentation be included elsewhere in the proposal.” Please clarify.

a: The professional compensation plan is evaluated under the Technical Capability Factor (see M3.1.2.1.3) not the Price Factor where Attachment 8, Table B-1, Representative Task Order is located (see M3.2). The Government will review the overarching professional compensation plan. The plan, in general, shall apply to professional positions listed in Section J, Attachment 4, Available Position Descriptions. The measure of merit for the professional compensation plan is outlined in Section M3.1.2.1.3.

b: L6.1.2 refers to specific pricing on the Representative Task Order and should be based on elements of the offeror’s professional compensation plan. The Government does not expect to review specific prices in the professional compensation plan.

2.4.5.6 HCWs will be required to record hours worked in the Defense Medical Human Resources System – internet (DMHRSi) system as required by the MTF.

Question: Can we use this time information as our time sheets? Is it verified by an on-site supervisor?

DMHRSi is a Government system. Please refer to Section C, paragraph 2.3.1.

2.4.17.4.4 Qualifying Documentation for Preliminary Government Minimum Requirements. The Contractor shall provide to the MTF Service Contract Manager the qualifying documentation on preliminary Government minimum requirements within 20 calendar days of receiving a Letter of Intent to Award (LOIA) or upon notice of a HCW requiring permanent replacement or substitution…

Question: Can this be submitted in the form of a resume or do you need copies of primary sourced licenses and certifications at this point?

The Government does not dictate the specific documents the offeror expects to submit as qualifying documentation to meet the preliminary Government minimum requirements. Please refer to the definition of preliminary Government minimum requirements in Section C, paragraph 1.4.42 and qualifying documentation in Section C, paragraph 1.4.46.

Question: How is the Task Order Process different in this solicitation from the method that is currently being used for this Program?

The process used in the current contracts is irrelevant to the task order process outlined in this RFP, which must be addressed in the offeror’s proposal. Please review Section C, paragraph 1.1.4 regarding the candidate approval component and the task order award component of the 2-step task order process.

Question: The contractor has 20 calendar days to submit a resume and the MTF has 7 business days to approve the candidate. We then have 30 days to submit the complete credentials packet. How long does the MTF have to approve a credentials packet after it is submitted by the contractor?

Upon award, the Government will transmit the task order to the Contractor on a DD Form 1155. The contracted HCW shall physically begin performance no later than 30 calendar days from the initial date on the period of performance in the task order for non-credentialied HCWs (see H.3h), unless a later date is specified in the task order, and no later than 60 calendar days from the initial date on the period of performance in the task order for credentialed HCWs (see H.31), unless a later date is specified in the task order.

Question [a]: At what point in this 30 and 60 day window do we receive approval by the MTF on the candidate’s completed packet? How long does it take the Government to process both non-credentialed and credentialed HCWs?

[b]: We also believe that employees should give notice to their current employer before starting a new job. Typical notice is between 2 and 4 weeks, depending upon the specialty. Candidates will not give notice until they are accepted and approved at the MTF. How is this factored into the time frame?

a: The length of time for MTF approval varies across the Air Force.

b: The Government does not dictate the external employment process.

H.24, Section g. Pertaining to Health Care Worker Average Rates

The Contractor shall use the average hourly rates in Table H-4, HCW Average Hourly Rates when determining the unobligated dollars for credentialed and non-credentialed HCWs. The Contractor shall use the loss duty hours for each type of HCW times the associated average hourly rate for each type of HCW in Table H-4 to calculate the unobligated dollars for the obligation rate percentage on the Contractor Performance Status Report (CPSR). The Contracting Officer may update these rates during the life of the contract.

Question [a]: How were these average hourly rates calculated?

[b]: Why isn’t the actual contract rate used for this calculation to get the real picture?

a: The Government calculated average hourly rates for credentialed and non-credentialed HCWs using average rates from FY10.

b: As outlined in Section C, paragraph 2.4.17.4.2.7, the Government chose to set the lost duty hours due to “delays in initial, replacement, and substitution fills as sufficient to represent the obligation rate percentage since actual contract rates are not releasable.

H.2, Task Order Procedures (Representative Task Order Only)

Section b. states “…by issuing a Letter of Intent to Award (LOIA) to the Contractor identified by the Program Management Office as the most advantageous to the Government.”

Question: Can the Government provide more detail regarding what is considered to be most advantageous? It is understood that a software program will assist in making this analysis (M3.2.6.4), however how is it ensured to be most advantageous while still not overwhelming any one awarded contractor?

The offerors awarded contracts are determined to be the most advantageous to the Government because they represent the best value from the Performance-Price Tradeoff source selection.

The allocation of RTO requirements is a separate function which allocates RTO requirements among the awardees that were determined to be the most advantageous to the Government.

H.2. Task Order Procedures (Representative Task Order Only)

Section c.1.c) discusses the validation process.

Question [a]: If the order is revalidated and the LOIA rolls from the first to potentially a third contractor, will the timelines begin over in fairness to the final contractor attempting to fill it?

[b]: Additionally, what happens if a task order is not validated? Is the entire requirement then cancelled?

a: The “third contractor” will be given an LOIA, which is referenced in Table C-1, Services Summary and H.2c.1c).

Question: Can the Government clarify when backfill is required (i.e. is it after 3 missed shifts or some other set frequency?) Or is this indicated at the task order level for each requirement?
Note: The word, “backfill” is not in the RFP.

The RFP references substitutions and replacements, which is outlined in Sections H.16 and H.17, and defined in Section C, paragraphs 1.4.50 and 1.4.57.

Question: What are the steps for the process discussed in this section and what are the potential consequences?

Contractors are on the Watch List because they fail to meet one or more acceptable quality level, which could impact their ability to get future contract awards. Please refer to H.3c.3.a).

Question: While it is noted there is one cost type line item for travel, this clause indicates it does not apply to small businesses or foreign governments. Because this is a small business set aside RFP, can this clause be removed, please?

The Government will not remove this clause.

52.215-11 Price Reduction for Defective Cost or Pricing Data – Modifications (OCT 1997) and 52.215-13 Subcontractor Cost or Pricing Data – Modifications (OCT 1997) AND

Page 110, 52.215-20, Requirements for Cost or Pricing Data or Information other than Cost or Pricing Data

Question: Page 46 indicates certified cost or pricing data is not required for the task orders. Please explain why these clauses are pertinent and if not, can they be deleted? Please confirm Certificates of Current Cost or Pricing Data are not required.

Although we anticipate adequate price competition for this acquisition, these clauses are necessary in the event that adequate price competition does not exist on the contract or associated task orders. If there is adequate price competition, Certificates of Current Cost or Pricing Data are not required.

When certified cost or pricing data are required, the contracting officer shall require the contractor to execute a Certificate of Current Cost or Pricing Data, FAR 15.406-2(a).

L.6.1.3 Pricing Information Requirements

Question: Can the Government clarify what circumstances an offeror shall be required to submit pricing data? Does the Government foresee the need for cost or pricing data when this solicitation is assumed to have adequate price competition?

Escalation Rates

Question: Can the Government clarify which index (or indices) are acceptable for escalation based on its independent analysis?

L-4 Client Authorization Letter

Question [a]: In the instructions, it states “…we request that a client authorization letter (similar to the sample below) be completed by any commercial customers identified in your proposal.” Shouldn’t that state that the contractor shall complete the letter and submit it to the commercial client?

[b]: Please confirm that the Government wants a copy of this letter to the client in the proposal response.

a: Yes. The RFP will be amended to read “…we request that a client authorization letter (similar to sample below) be completed for any commercial customers…” b: Please review the note in Table L-1, Proposal Organization.

Past Performance Tool question [a]: may go into the system to view the requirements of the Past Performance Tool and then take the necessary time to develop the answers to those questions as presented in the tool without a time penalty or some other sort time constraint on the information we intend to input into the system?

[b]: Is there a potential to submit accidentally before we are finished inputting necessary information into the system if we leave the system after viewing?

a: The past performance tool is not a web-based application but a Microsoft Access Database. Please refer to paragraph 4 in “Downloading the PPI Template” in Attachment L-5.

The CLASS FedBizOps announcement indicates the overall CLASS objective is to fill permanent staffing shortages in MTFs. It is understandable that your goal of continuity and quality rather than spontaneity should result in better results. However, while the requirement to provide qualifying documentation for candidates within 20 calendar days is attainable for some locations and service categories, it is only attainable for many others through a short-term, locum tenens placement due to realities in the marketplace. The Medical Group Management Association’s (www.mgma.com) survey of 192 physician recruitment departments across the nation found that the median number of months to place a pediatrician is 6 months, an OB physician 8 months, an FP physician 6 months, and an internal medicine physician 7 months. The 20-day requirement seems incongruent with the requirement to ensure staff remain in positions for at least 180 days, since locum tenens placements (which companies will be driven to with a shorter timeframe to recruit) are inherently short term. Turnover rates, which you would like at 25% or less, would be impacted by use of locums staffing. Will the government reconsider the 20-day requirement with the purpose of giving vendors the time needed to recruit and place long-term, permanent professionals to fulfill MTF needs? At a minimum, we recommend increasing the 20-day requirement for the highest demand positions and geographical areas with known shortages.
[Note: Offerors will not be required to recruit and place health care workers within 20 days. Please refer to Section H.3c.5.a)1) for the submission of qualifying documentation when recruiting candidates and H.3h and H.3i for the placement health care workers. Section C, paragraph 1.1.5 provides a review of the Contractor activities.]

Please provide the anniversary dates of the incumbents for calculation of PTO under the Service Contract Act.

In accordance with FAR 52.222-41(n) the Government will turn over a certified list of the names of all service employees on the Contractor’s or subcontractor’s payroll during the last month of contract performance. Such list shall also contain anniversary dates of employment on the contract either with the current or predecessor Contractors of each such service employee.

Reference H.1.b and H.2. Representative Task Orders – Are the RTO requirements for positions currently on other contracts, such as Federal Supply or 8(a) awards, and filled by incumbents? If so, to what extent, all of them?

It is our understanding that all of these requirements are filled by incumbents on the current Clinical Support Services Contracts. The contract companies are identified in Table B-1, Representative Task Order.

As provided the Excel Spreadsheet is 470 pages. Currently pages are formatted to print on 8-1/2 X 11 inch sheets. Recommend that spreadsheet be reformatted to print on 11 X 17 inch paper (landscape) with rows 1-5 headings and columns A and B on the left side of each page. Also recommend that spreadsheet be set up to print at 75% of normal size. This formatting allows offerors to provide the required information on 72 pages and assists evaluators with the pricing proposal evaluation.
Volume III, Tab 2 in Table L-1 Proposal Organization will require submission of Electronic Version only.

RFP Section L para L.3.4.8 -- 2nd paragraph states “Legible tables, charts, graphs and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, timelines, internal processes, plans, and monitoring and tracking systems.” However, at the end of this paragraph it states: “For tables, charts, graphs and figures, the text shall be no smaller than 12 points using Microsoft Word, Times New Roman font.

Section L instructions appear to be in direct opposition to each other and requires clarification. Use of a 12-point font in tables, charts, graphs, and figures limits offerors ability to display detailed information, particularly with regard to process flows and organization charts. Will the Government please revise the Section L instructions to allow graphics with fonts size no less than 9 point.

The Government will modify the language in L3.4.8 on text limits for tables, charts, graphs and figures as follows, “For tables, charts graphs and figures, the text shall be not smaller than 9 points using Microsoft Word, Times New Roman font."

Section L para L2.3.4.14 states that MS Office 2007 files (docx; xlsx, pptx, etc) will be used for the softcopy submission which has a preference over the paper copy documents submitted.

The RFP states “..and that the softcopy will take precedence over the hard copy.” Offerors have limited control over pagination on different computers. MS Word pagination on two different computers is often varies. The requirement also states: “Submit portable document format (PDF) documents using Adobe Acrobat 9.0 only. DO NOT submit any documents in PDF format that are copied as ―images.” This indicates that offerors are permitted to submit documents in *.PDF format, thereby eliminating pagination issues.

Does the reference *.PDF format mean offerors may submit narrative files in *.PFF and ensure hard copy and electronic copies are identical? Please clarify.

[Note: The Government believes the correct reference in the question is L3.4.14 and the response is based on this reference. The word, "softcopy” is not in the RFP. The RFP actually states, “the electronic version shall take precedence and will be used for evaluation of the offeror’s proposal”.]

As stated in Section L3.4.14, offerors must submit portable document format (PDF) documents using Adobe Acrobat 9.0 only.

According to Attachment 8 Table B-1, Representative Task Order spreadsheet, labor escalation is applied for all positions (Exempt and Non-Exempt). Given that FAR 52.222-43, which restricts the escalation of Service Contract Act labor, is referenced in the RFP, is it the Government’s intention that offeror’s escalate non-exempt labor rates?
FAR 52.222-43 does not allow for escalation of labor rates for SCA-covered positions.

RFP Section 1.4.56 SUBSEQUENT GOVERMENT MINIMUM REQUIREMENTS: The minimum HCW requirements to fully qualify HCWs for their placement in an MTF include, but not limited to, health; immunizations; drug screening; complete background checks; credentialing; and security. The Government uses the subsequent Government minimum requirements to ensure HCWs are fully qualified to perform in an MTF.

[a]: Can Bidders be assured that all tasks (i.e. physicals) and all tests (i.e. drug tests, TB tests) for subsequent qualifications listed will be performed on site at the MTFs at Government’s expense?

Regarding the National Escalation Factor – Can Bidders be assured that should our actual cost experience exceed our rates as adjusted by the National Escalation Factor quoted in our proposal, that a Rate adjustment can be requested under FAR 52.222-43 to include providing payroll and accounting records that support the claimed deviation above the National Escalation Factor?

A price adjustment can only be requested as outlined in Section H.11.

Does the government realize that Small Businesses will have a hard time responding to at least 85% of some 300 TOPRs a year to win about 30 awards a year (1/10 of the total awards) and that this places a very high overhead on Small Businesses to continuously respond with few results? Perhaps Government could selectively send out TOPRs to lower this overhead cost?

The CLASS contracts are indefinite delivery/indefinite quantity contracts as stated in Section L3.1. The Government has considered this suggestion and declines to change the RFP.

Also PWS Section C para 1.1.2 and Figure C-1. Indicates a flow process where by resumes may be required for the initial TOPR submission.

DISCUSSION: We understand that the TOs may or may not be issued to require a candidate resume for USAF consideration (acceptance/rejection) as a part of TO award. However, we assume that the TOs when issued would specify if resumes are required prior to award. Likewise, one would assume that the RTO (Section J, Attachment 9) should have said if resumes were a part of an offerors proposal response to RFP L5.1.2.3.2 response. Also because the RTO documentation is located inside the Technical Capability response a two-page ‘representative’ resume for two positions should clearly state that the RTO resumes are excluded from the total 50 page limit for Volume II – Technical Capability.

QUESTION [b]: Will the Government please clarify the RFP instructions on:

[i]: Attachment 9 response must include resumes not to exceed two (2) pages

REFERENCE: RFP Section L7.1.1 Early Proposal Information states: “The Government requests offerors to submit their past performance volume seven (7) calendar days prior to the date set for receipt of proposals.”

DISCUSSION: The reference states that the Government “requests” but does not mandate the early submission nor does it clearly specify what constitutes the 7-day early submission as a complete package. The RFP is silent on the early submission being considered late IAW the FAR clause if not delivered on 12 May 2011. Paper copy and CD-Roms or just the paper copy only? It would make sense that the questionnaires should be returned concurrent with when the Government expects to begin Past Performance evaluations. However, the RFP indicates that they are due along with the proposal due date, currently set for 25 May 2011. Industry needs to understand what pieces of Volume IV Past Performance should be included in the early submission package.

QUESTION: Will the Government please clarify the missing elements associated with the early past performance submission:

a. Volume IV Past Performance due 12 May 2011 in 3 Hard copies (Original, 2 hard copies) and one CD-Rom (electronic copy)

b. Volume IV Past Performance submitted after 12 May is considered late IAW FAR Clause 52-214-7: Late Submissions, Modifications, and Withdrawals of Bids

The spreadsheet in Attachment 8 Table B-1 (Representative Task Order) has a column that lists the Central Requirement. What does the Central Requirement number represent and how does this impact the total number of FTE listed as being required?

Central requirements on the RTO refer to centralized requirements, which cover "multiple MTF locations" as outlined in Section H.15b. As outlined in Section H.2(a), the central requirements on the RTO may be issued and administered at the individual base or through the Enterprise Sourcing Group.

RFP Section H.8 – Liability Insurance (page 56). The general liability insurance coverage required is at least $500,000 per occurrence. Under existing contracts with the AFMS, we have seen general liability coverage requirements of at least $1M per occurrence. Would you consider changing this insurance requirement to $1M per occurrence?

Section L.7.1.5.2 References (page 125) states that the offer is to enter each past performance reference into the provided Access tool and print. Proposal instructions on page 115, Section L.3.4.8 require the Offeror to consecutively page number the volume. Additionally, the proposal instructions require a Master Table of Contents. However, the Access tool does not allow the Offeror to page number the past performance to include in their proposal response. Without page numbers on the past performance print outs from the tool, what should the Offeror use in the table of contents and the past performance volume for numbering?
The Government does not expect the pages generated from the past performance tool to be numbered.

RFP para 1.3 states: The Contractor shall provide all tools, labor, equipment and materials required to place the HCWs within the MTFs, in accordance with the terms and conditions of this contract and all related task orders. Could the Government please clarify if the contractor is expected to provide any equipment used in the performance of duties required by this contract?
The reference discusses equipment the Contractor needs to place a health care worker. Government furnished equipment is outlined in Section C, paragraph 4.2 during health care worker performance.

RFP para 2.2.1.2.2 states: The chief of the medical staff, or his/her representative, will define the clinical privileges for all credentialed HCWs during the credentialing process. All credentialed HCWs shall be able to obtain independent privileges for practice within the scope of care through the applicable MTF Credentials Committee. Independent privileges allow the health care worker to practice independently within the scope of care without supervision such as required under supervised privileges. Although we understand that obtaining clinical privileges is dependent on credentialing package, will the Government clarify any unique or site specific requirements to obtaining clinical privileges in the Task Order Proposal Request?

RFP para 2.4.17.4.4.5 states that the contractor must provide supporting documentation that an initial background check was performed on the candidate. Can the Government please provide more detail of what is expected on the background check and what information the supporting documentation must show?
The definition of the initial background check is provided in Section C, paragraph 1.4.28.

The details of the background check are inherent in the process established by the offeror as outlined in Section M3.1.1.3.1.
