Statement of Work

W.O. # 35921, Chapel Kitchen

7 May 2019

1. General Requirements

a. Contractor is to provide all labor, materials, services, oversight and direction to complete the design and construction of the requirements identified in this SOW and attached materials to renovate the Fellowship Hall in the Base Chapel into a functional dining area. Detailed requirements are listed below but in general this project will design and construct a dining area. All design and construction will be required to adhere to the current standards set forth by Unified Facilities Criteria

(UFC.), 2015 IBC, IMC, IPC, IECC, NEC, and other applicable standards as referenced in this Statement of Work or Manufacturer’s criteria.

b. All work will be compliant with the Architectural Barriers Act & UFC 1-200-01.

c. All materials specified by the Govt. may have approved equals.

d. The total Period of Performance (POP) will be 150 days. This will include initial assessments, design development, submittals, review periods and construction.

e. The contractor must have an approved 65% design before commencing new construction. A color board with physical samples of finish materials will be included.

f. The 95% design will be submitted within 30 days of 65% design approval. This will include a final finish schedule.

1) Maps of the project location and utility locations are attached. Drawings indicating desired layout and material recommendations are also attached.

The Contractor shall field verify all pertinent conditions/locations.

2) Design submittals shall be supplied at the 65% and 95% stages for

Government review and comments. A pre-design kick-off meeting will be held prior to the start of design and is to be attended by Mountain Home AFB personnel, design firm, and construction contractor. All comments made by the government during submittal review, within this scope of work, shall be incorporated into the design.

3) Provide a complete, stamped code analysis reviewed by a registered architect, to include life safety.

4) The Contractor shall identify and obtain all required permits from Mountain

Home AFB, federal, state, and local agencies.

5) Material storage areas and contractor staging areas will be selected during the pre-construction meeting.

6) Disposal of solid waste must comply with Environmental Protection Plan as attached.

7) During the progress of the work, the contractor shall keep a careful record at the job site of all changes and corrections from the layouts shown on the drawings, if applicable, regardless of whether they are a part of the construction effort or not. This shall include any hidden or subsurface data discovered during construction. The contractor shall enter such changes and corrections on a copy of the deliverable Final Construction Drawings and provide the Contracting Officer (CO) with a copy of the updated drawings.

8) The as-built drawings shall indicate all physical changes to the construction area and the actual location of any subsurface utility lines. The drawings shall also show, by offset dimensions to two (2) permanently fixed surface features, the end of each run including each change in direction in sufficient detail that anyone may determine the location of these lines and appurtenances in the event the surface openings or indicators become covered over or obscured. The drawings shall also note the average depth below the surface of each run.

9) The contractor shall submit two (2) full sets of 11”X 17” reproducible as-built drawings that are neat, to scale as indicated on the drawing, and clearly marked to show all variations or changes between the construction as-builts and that indicated or specified to the CO at final inspection. The drawings shall show any hidden, subsurface, or corrected layout information that the contractor discovered during construction regardless of whether they were a part of the construction effort. The contractor shall also provide two (2) copies of the drawings on a writable CD. The disk drawings shall be in the native file format used by the 2014 version of AutoCADD software systems for Windows and capable of loading without conversion or editing. The

CADD drawing format (i.e. level/layer structure, symbology, etc.) shall conform to the CADD standards used by the Base Civil Engineer

Engineering Assistant element. The Government’s project manager will provide the contractor with a copy of these standards and any updates as they become available throughout the life of the contract. All drawings shall conform to standard drafting practices and include such supplementary notes, legends, and details as required. Dimension all utilities from a known point, or an existing permanent object, to the centerline of the ditch or structure.

The legibility and clear portrayal of the as-built construction on the marked prints (hard copy) are subject to the approval of the CO. The Government will review the as-builts for accuracy and if found in error, will return the drawings to the contractor for further revision. The contractor shall revise drawings and resubmit to CO for approval.

10) The contractor shall furnish two (2) copies of operation and maintenance manuals for any product installed in the construction project. The manuals shall contain complete instructions for operation, inspection, testing and maintenance of the system and shall include part breakdown diagrams. If applicable, the manuals shall also contain complete wiring schematics and diagrams. Organize operation and maintenance data into suitable sets of manageable size. The contractor shall provide the CO all paperwork that was shipped with or attached to the product or material including:

Installation or maintenance instructions for the product or material.

Part number listing

Care and/or cleaning instructions

Removing and replacing information

Warranty information

Phone number for information

Operator Manuals

g. Environmental: All work shall be in accordance with Mountain Home AFB

Temporary Environmental Controls updated 19 January 2018. This specification is attached.

1) Submittals: All the submittals will be provided to the CO for review. The contractor/designer will prepare a comprehensive submittal register.

2. Site Construction

a. Landscaping: Restore areas affected by utility installation to prior condition.

b. Concrete: Concrete shall have a minimum 3500 psi 28-day compressive strength.

c. Openings: Door hardware shall be “Best” brand compatible

d. Finishes: Colors to be selected by customer from manufacturer’s standard pallete

e. Fire Suppression: Design to be performed by NICET Level 4 technician

1) Kitchen sink to be two basin with tall gooseneck faucet

f. HVAC: Retain existing units

g. Electrical: Design/provide electrical service as required

1) LED troffers to be dimmable

h. Electronic Safety and Security:

1) Design/install horn/strobe devices as required by code.

a. Notification devices must match existing facility

2) Design/install manual pull stations at each exit as required by code.

i. Earthwork:

1) Bed/backfill trench IAW with ISPWC Section 305 & 306

a. Restore turf, sidewalk, sprinklers, etc. to prior condition

A&E Design Submittals

1. Normal Government review time of submittals for each phase shall be seven (7) days.

2. Submit two (2) hardcopies copies of drawings and two (2) electronic copies of drawings and specifications for the 65%, and 95% submittal requirement. Submit two (2) electronic copies of the drawings, specifications and AutoCAD files for the

100% submittal and (2) hardcopies. All submitted drawings will be 11”x17”.

a. Preliminary design analysis for civil, structural, mechanical, and electrical work.

b. Outline specifications defining specifications to be used.

c. Construction prepared at working scale which will include the following:

1) A vicinity map and location plan showing geographic location and relation of the facility to other existing buildings, roads, streets, etc.

2) Floor plans at scale of not less than ¼” per foot, showing dimensions and functional arrangements of all areas, including corridors, exits, and utility spaces properly related to exterior access, service areas, etc.

3) Exterior elevation, including planned materials, in sufficient detail to portray the exterior appearance.

4) Site and building electrical and mechanical drawings showing general layout of existing and proposed work.

3. The 95% design submittal package shall contain two (2) hardcopies and two (2) electronic copies of drawings and specifications, and shall include the following:

a. Schedule of Drawings.

b. Schedule of Construction Material Submittals using a completed AF Form 66.

c. Construction Drawings.

d. Design Analysis and calculations with manufacturer cut sheets of selected products assembled in order of the submittal register.

4. The 100% design submittal package shall contain two (2) hardcopies and two (2) electronic copies of drawings and specifications, and shall include the following:

a. Schedule of Drawings.

b. Schedule of Construction Material Submittals using a completed AF Form 66.

c. Final Construction Drawings stamped with P.E. stamp, signed and dated by the responsible engineer or architect. Additionally, provide CD-ROM copies of drawings in *.dwg or *.dxf format compatible with 2014 edition of AutoCAD.

d. Construction timeline. Include as a minimum the following estimated dated milestones: mobilization, construction schedule (CPM) submittals, long lead time equipment and material items, utility outages and any additional items that might hinder normal operations at the project site, pre-final punch, commissioning, and construction completion.

e. Design Analysis and calculations with manufacturer cut sheets of selected products assembled in order of the submittal register.

f. Completed DD Form 1354, Acceptance and Transfer of DOD Real Property

5. SUBMITTAL SCHEDULE

Submittal Schedule and Requirements: The schedule for delivery of Final

Documents is in calendar days after notice to proceed.
