The purpose of this amendment is to correct Amendment 0003 and incorporate changes to the provision 52.252-3, Alterations in Solicitation.
1. Amendment 0003 correction. Block 11(a) of the SF30 was left blank; it should have been filled in with "1";
thereby reading "…returning 1 copies of the amendment;".

(b) Submission of offers. In addition to this paragraph's (1) through (11), the following are added:
(12) Purpose: The purpose of these instructions is to prescribe the format of proposals and describe the approach for the development and presentation of proposal data. They are designed to ensure the submission of information essential to the understanding and comprehensive validation of proposals. Offerors are cautioned to follow the detailed instructions fully and carefully, as the government can make an award based on the initial offers received, without discussion of such offers.
(13) Proposal Format: Each proposal shall be submitted in three (3) volumes. Failure to comply with these instructions may result in the offeror’s proposal being summarily rejected. Any commitments made in the proposal shall become a part of the resultant contract.
Part I – Technical Proposal
Part II – Past Performance Information and
Part III –Solicitation/Price Proposal
(i) Page. The proposal page size may be 8½" x 11", except for fold-outs used for charts, tables or diagrams, which may not exceed 11" x 17". A page is defined as one face of a sheet of paper containing information. When both sides of a sheet display printed material, it may be counted as two (2) pages. Each page may be counted except cover pages and table of contents/glossary. If exceeded, the excess pages may not be read or considered in the evaluation of the proposal. Excess paper copies may be destroyed (except within the original proposal), which may render the Offeror’s proposal "technically unacceptable."
(ii) Font/Binding. Pages shall be typed with a font pitch size 11, Times New Roman type, with one-inch margins. Each part (volume) may contain a detailed table of contents to delineate the sections and subparagraphs contained therein. All required documentation may be page numbered and bound in accordance with the table of contents submitted. Each part (volume) should be separately bound. Staples may not be used. Consolidated parts are acceptable provided tabs separate them. A cover sheet may be found in each part (volume), clearly marked as to the part number(s), title, copy number, solicitation identification and the Offeror’s name. Past Performance (Part II) may not include price or pricing information. Elaborate formats, bindings or color presentations are not desired or required.
(14) Submission Requirements.
(i) Submit one original and five (5) copies of Part I-Technical Proposal and Part II-Past Performance
Information. Submit one (1) original of Part III-Solicitation/Price Proposal. Submit one (1) electronic copy of all
Parts I, II & III, in accordance with the following Proposal Organization Table (also reference para (15):

(15) Electronic Documents. The recordable, virus-free data compact disk (CD) shall contain the entire proposal (all
Parts) and located in the original binder of Part III- Solicitation/Price Proposal. Label the CD with the solicitation number, title, Offeror’s name and marked Source Selection Sensitive/For Official Use Only. Failure to provide an electronic copy as specified may render the proposal nonresponsive. Within the CD, use a separate electronic file for each part with associated exhibit, annexes, and attachments to permit rapid search and location. Files may not be compressed. Submit electronic proposal documents in a format readable by Microsoft (MS) Office Word 1997-
2003 or 2007, 2010 or 2013, MS Office Excel 1997-2003, 2007, 2010 or 2013, and MS Office Power Point 1997-
2003, 2007, 2010 or 2013, as applicable. In the event of discrepancy(s) between the hard copies and electronic copies of a proposal, the hard copies shall take precedence and be used for evaluation. The completed documents should be marked Source Selection Sensitive/For Official Use Only.

(17) The CO may control exchanges of source selection information between Government and Offerors. The
Government may use email to transmit such information to Offerors only if the email can be sent and received encrypted and includes "Source Selection Information – See FAR 2.101 & FAR 3.104" in the subject line.
Otherwise, the Government may transmit source selection information via fax or other carrier.
(18) Part I – Technical Proposal:
1. The technical proposal shall not make reference to price data. It will be evaluated as acceptable or unacceptable and will consist of six (6) Factors. ALL factors must be deemed acceptable in order to be technically acceptable.
2. Technical Proposal Requirements: The technical proposal will primarily describe the capability of your organization to perform the requirements of the contract, it shall be specific and complete. The proposal shall not merely offer to perform work in accordance with the Performance Work Statement, but shall outline the actual method proposed as specifically as possible. Non-conformance with the instructions provided may result in an unacceptable technical evaluation.
3. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal should not simply rephrase or restate the Government's requirements but, rather, shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of their facilities or experience and will base its evaluation on the information presented in the offeror's proposal.
4. Your technical proposal shall, at a minimum, contain the information specified below in accordance with the following general outline:

Factor 1 The offeror must submit a detailed approach/plan to perform Operations and Maintenance (O&M) of the equipment which includes all items as specified in the PWS Section 1.3 (inclusive), Appendix 5.2, Items to be
Maintained by the Contractor, AND the Offeror’s draft maintenance support plan which shall contain the minimum requirements as listed in PWS 1.3.15.1.
Factor 2 The offeror must submit a detailed plan that describes how the Offeror will complete work orders within time limitations listed in Attachment 1, PWS, para(s) 1.3.4.3.1, 1.3.4.3.2, 1.3.4.3.3, and 1.3.6.
Factor 3 The offeror must submit a detailed plan that describes how the Offeror will simultaneously perform the four major tasks and the estimated quantity of tasks listed in the PWS Appendix 5.1 as well as the tasks listed in paragraphs 5.1.2 (inclusive).
Factor 4 The offeror must submit a detailed plan describing how the Offeror will maintain the performance thresholds listed in the PWS paragraph 2.0.
Factor 5
The offeror must submit a detailed summary description of personnel and vehicles, tools, materials and equipment to meet the scope of work specified in PWS paragraph 1.1. The description shall include the following areas:
Personnel – which indicates a sufficient number of on-site personnel in each category required to meet all contractual requirements in PWS 1.2.7.1 and show adequate contractor availability as specified in the PWS paragraph 1.3.4.1., to include the following for each:
o labor code categories as specified in solicitation Attachments 4.1 and 4.2, and o short occupational title(s) of personnel specified in solicitation Attachments 4.1 and 4.2
Vehicles, Tools, Materials, and Equipment – The description shall show the Offeror’s approach to providing the adequate contractor-supplied vehicles, tools, materials and equipment to maintain the secondary crash systems, hospital and base key systems, etc. listed in PWS Appendix 5.2.
Factor 6
The offeror must submit a draft quality control plan as stipulated in PWS paragraph 1.3.15.18 and which describes elements of their total quality control program (PWS paragraph 4.1). This plan shall address, as a minimum, how the Offeror will ensure the required performance objectives listed in the PWS paragraph 2.0 are to be met, reflects the required elements listed in PWS paragraph 1.3.15.18 and explains the Offeror’s customer feedback/corrective action program (PWS paragraph 4.1). The plan will have a section titled Mission Essential
Contractor Services and will cover all items in accordance with DFARS 252.237-7024 Notice Of Continuation
Of Essential Contractor Services.
(19) Part II – Past Performance:
Past Performance section is limited to a maximum of the three (3) most recent/relevant contracts, see paragraph
(b)(14) Table 1 for page limitations. The 2 page Attachment 5, Past Performance Information Sheets (PPIS) are not included in that page limitation and each PPIS should reflect one of the three (3) most recent/relevant contracts used for Past Performance. The page size shall be 8 ½ x 11 inches. Pages shall be typed with a minimum of font size 11, Times New Roman type, with minimum 1-inch margins. Contractor's Past
Performance shall consist of the following two (2) items:
a. Pertinent Information: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrates customer satisfaction with the Offeror’s overall job performance and quality of the Offeror’s completed contract services for the same or similar type contract, Base
Telecommunication Services, referenced in this solicitation. The Offeror may provide the amount of time the
Offeror has been doing this type of work and a description of the work performed. In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems affecting performance (such as numerous incidences of re-performance of work, discrepancies, etc.).
b. Performance Data (Use Attachment 5, Past Performance Information Sheet (PPIS)): The Offeror should select contracts from previous employers/customers which shows the quality and extent of the Offeror’s performance deemed relevant to the requirements of this solicitation. It is the Offeror’s responsibility to ensure all past performance data submitted is current and correct so that the Government can perform a proper evaluation. Provide no more than three (3) of the most recent and relevant contracts performed for federal agencies and commercial customers.
Recent is defined as within the last three (3) years, (contract must be currently on-going or have a start date of October 2013 or more recent).
Relevant is defined as contract work in the area of Base Telecommunication Services as identified in the scope of work of the Performance Work Statement (Attachment 1 to this solicitation).
(20) Part III – Price Data:
This section of the proposal shall include only the following:
a. The signed Standard Form 1449, Solicitation/Contract, returned in its entirety along with all required fill-in documents. (Do no return attachments, except Attachment 2, PIRRM Pricing Schedule completed).
b. Complete all blanks for unit and total prices for Base and Option Years.
c. Any, signed, solicitation amendments thereto.
Addendum to 52.212-2, Evaluation—Commercial Items (OCT 2014)
In addition to paragraphs (a) through (c), the following are added:
(d) Basis for Award: This acquisition will utilize the Lowest Price Technically Acceptable (LPTA) technique to make a best value award decision. The application of the LPTA technique in the contract award selection and approval process is as follows:
(i) The Government will review the two lowest priced offers and if both are deemed technically and performance acceptable, award will go to the lowest priced offeror and no further proposals shall be reviewed.
If either of the two lowest priced offers are not deemed technically and performance acceptable, the next lowest priced offer shall be reviewed for technical and performance acceptability. If this offer is not deemed acceptable, this process shall continue until two offerors are deemed technically and performance acceptable or until all are reviewed. Note: In accordance with FAR 52.212-1(g), as prescribed in FAR 12.301(b)(1), "The government intends to evaluate proposals and award a contract without discussions with offerors" (except clarifications as described in FAR 15.306(a)). Therefore, each initial offer should contain the offeror’s best terms from a cost or price and technical standpoint. However, the government reserves the right to conduct discussions if later determined by the Contracting Officer (CO) to be necessary." The CO may request offerors of marginal proposals to submit clarifications of areas identified by the technical proposal evaluation team. In initiating a request, the CO will set an appropriate time for submission of such information. Late submissions will be handled in accordance with FAR 52.212-1(f) "Late Submissions". The clarification information will be incorporated as part of the proposal.
(ii) The lowest priced technically acceptable proposal will be deemed the successful offeror and award shall be made to this offeror, subject to a positive responsibility determination.
(e) Technical Area:
(i) A technical review team composed of key government personnel will use their technical skill, knowledge and experience to thoroughly review the adequacy of proposals. Each proposal will be evaluated by application of the assessment criteria to each factor in the technical proposal. Only those proposals determined technically acceptable will be considered for award. Each Factor will be considered acceptable or unacceptable as defined below:
Acceptable: Proposal meets the minimum requirements of the solicitation.
Unacceptable: Proposal does not meet the minimum requirements of the solicitation
(ii) Offerors must pass all Technical Factors in order to be deemed technically acceptable:
(f) Part I – Technical Proposal:

Factor 6
This factor is met when the Offeror provides a draft quality control plan as stipulated in PWS paragraph

This plan shall address, as a minimum, how the Offeror will ensure the required performance objectives listed in the PWS paragraph 2.0 are to be met, reflects the required elements listed in PWS paragraph 1.3.15.18 and explains the
Offeror’s customer feedback/corrective action program (PWS paragraph 4.1). The plan has a section titled
Mission Essential Contractor Services that covers all items in accordance with DFARS 252.237-7024 Notice Of
Continuation Of Essential Contractor Services.
(g) Part II – Past Performance:
(i) Past performance shall be evaluated in accordance with FAR 15.305 and DFARS 215.305. The comparative assessment in FAR 15.305(a)(2)(i) does not apply. Past performance will be rate on an "acceptable" or
"unacceptable" basis as defined below. Therefore, Past performance will be rated acceptable when the Government has a reasonable expectation that the offeror will successfully perform the required effort, based on review of the offeror’s performance record or the offeror’s performance record is unknown.
Acceptable: Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown. (See note below.)
Unacceptable: Based on the offeror’s performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.
Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305
(a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, "unknown" shall be considered "acceptable."
(ii) There are three aspects to the past performance evaluation.
a. The first is to evaluate whether the offeror’s present/past performance is relevant or not relevant as defined in Addendum to 52.212-1, Instructions to Offerors—Commercial Items (OCT 2015), Paragraph (19) Part II

(iii) Offerors present/past performance must be recent, relevant and successful to be deemed acceptable and considered for award.
(iv) The Government reserves the right to use other data than the questionnaires. This may include data gathered from Offeror-provided references/experience/ information sheets, questionnaires and through the use of interviews. Furthermore, the Government may use data independently obtained from other Government and commercial sources; other customers known to the Government; contractor performance evaluations (e.g., Present/Past Performance Information Retrieval System (PPIRS) information, etc.); interviews with
Government customers, and, if applicable, commercial clients; and any other source that may have relevant and useful information.
(h) Part III – Price Area/Data
(i) Price Area – The offeror’s proposal price will be determined as follows:
a. First, by multiplying the quantity times the unit price for all the monthly maintenance CLINs (Contract
Line Item Numbers) (CLINs x001 & x002) for a subtotal. This includes these CLINs for the base period, all possible option periods to be exercised under FAR clause 52.217-9, and/or any possible extension under FAR clause
52.217-8 for six (6) months.
b. Second, by totaling a random selection of prepriced line items/product identification (PID) on the pricing schedule Attachment 2, Purchase, Installation, Relocation, Removal and Miscellaneous (PIRRM) for a second subtotal. The PIRRM has approximately 650 PID #s and the Government will evaluate a percentage of each year (base and options). The CLINs will be evaluated as described in the Table below for each year (base and options, as applicable) and combined together for an overall contract value.

*once awarded, CLINs x003-x004 will be prefilled with the Government estimated annual amounts for the projected year's works orders; PIRRM will be updated with Contractor's pricing.
c. The first and second subtotals (ref para (a)&(b) of this section), added together, will become the offeror's total estimated contract value for evaluation purposes.
