Contracting officer verification and project manager signatures are a requirement in order to process this document failure to do so will result in the document not being processed. This form is to be submitted a minimum of FIVE (5) days prior to the date that access is required. Contractor hours are Mon-Fri 0600-1800 unless otherwise specified above. Ensure that all required information is properly verified before submitting(no nicknames ONLY full legal names). Drones/UAS reporting: If spotted on the installation immediately report to BDOC (406) 731-3895. Include colors/markings, type (plane, rotary), location, flight path, payload (if applicable). DO NOT TOUCH grounded craft. The personnel listed have been propertly vetted to perform official duties on Malmstrom AFB. The personnel lsited have recieved AT Level I and UAS training IAW with DoD, AFI , and local policy. Contractor vehicles: All large contractor vehicle, (e.g. panel van, semi-truck, tractor, trailer, dump truck) that a contractor drives or tows on the installation. Small contractor passenger vehicles exempted from vehicles searches are cars, pickup trucks, SUVs, etc. Contractors will enter the installation through the 10th Ave North Gate. Contracting officer has verified the requirements for the following personnel to be granted access to 341 MW MAF's and LF's. Allow personnel within this CEAL to be authorized access to the required areas IAW DoDM S-5210.41_AFMAN 31-108 and AFMAN 31-101. This CEAL is a supporting document for official contractual duties in the missile field or the WSA. PROJECT MANAGERS/COMPANY MANAGERS ARE REQUIRED TO COLLECT THE DBIDs CARDS FROM THOSE EMPLOYEES THAT ARE NO LONGER WITH THE COMPANY AND TURN THEM INTO THE VCC. INFORMATION CONTAINED HEREIN IS PERSONAL AND WILL NOT BE DISCLOSED TO THE PUBLIC WITHOUT CONSENT OF THE INDIVIDUAL AFI 33-332, AIR FORCE PRIVACY ACT PROGRAM APPLIES.

1. The squadron requesting the personnel to be vetted.
2. Company: Main company of the contract. If multiple, type the company who the contract is with.
3.Subject: Name of the project.
4. Contract #: Contract Number for this project or name of the contract.
5. Effective Date: The first day the individual(s)s will need access to base or the MAF/LF.
6. Expiration Date: The last day the individual(s) will need access to base or MAF/LF.
7. Check as needed:
DBIDS Badge : Check this if the individual(s) will be working on the base and need access to enter through 2nd ave gate or 10th ave gate for work purposes.
WSA: Check this if you need it as a supporting document for the WSA and need the VCC to send you a copy. Also make sure to check DBIDS badge if the individual(s) needs base access to get to the WSA.
MAF/LF Only: Check this if you need it only as a supporting document to the MAFs/LFs. Note: If the individuals need access to the base please ensure you also check DBIDs Badge.
8 and 9: Contractor, Project/ Company manager name and office number: This information is required to inform the point of contact when the CEAL has been vetted.
