Demo Dorm 631

Project No. NZAS 23-0030

Statement of Work

Date: 07 January 2025

1.0. GENERAL: The Contractor shall perform all services necessary to accomplish the demolition of Dorm 631. This installation shall be accomplished in accordance with the (UFC Unified Facilities Code) specifications, the associated drawings, and additional requirements of the Statement of Work as discussed in the following paragraphs. The contractor shall comply with pertinent sections of the UFC specifications in relation to the work identified in the project, regardless of whether the specific sections are referenced.

2.0. REQUIREMENTS: The Contractor shall labor, materials, and equipment associated with the design of the demo of the entire building including exterior improvements shown on the attached drawings, topographic site survey, asbestos and lead paint survey investigation, cutting, removing, and capping utilities, backfill, compaction, grading, seeding, providing plans and specs and all other incidental items necessary to complete the work. Comply with current Air Force design standards, Headquarters Global Strike Command design directives, Malmstrom Air Force Base standards, and referenced publications.

The Contractor is being provided with two 10% conceptual drawings to show the intent of the project.

The Contractor shall use these drawings to provide final construction drawings IAW the referenced standards.

2.1. The Contractor shall provide the 341st Engineering Flight with the following for approval prior to proceeding.

2.1.1. The Contractor shall procure the services of engineers and architects to design the demolition and site restoration. The Contractor shall provide the 341st Engineering Flight with 35%, 85%, and 100% Construction Documents for review, comment and approval prior to proceeding. All construction submittals must be reviewed by 341st Engineering Flight and approved by the Contracting Officer prior to purchase of materials and demolition.

2.1.2. The attached Division 01 specifications in WORD are to be used and modified as needed to construct the project. If changes to the specifications or additional specifications are required, it is the responsibility of the Contractor to provide and edit them. All changes and additions to specifications shall be submitted for review and approval to the Contracting Officer.

2.1.3. The Contractor shall be provided with a Submittal Schedule in Excel for completion of items to be submitted prior to construction.

2.1.4. The Contractor shall be provided with electronic format of a Project Schedule Form 3064 and Progress Report Form 3065. The Contractor shall use these forms during the duration of the project to provide information to the Government.

2.1.5. Asbestos surveys were done in 1995, 1997, and 2005 and are attached to this SOW. The Contractor shall base his bid on abatement of the materials found to be asbestos containing in these reports. The contractor shall hire a licensed abatement contractor to perform the work and dispose of the materials in a legal manner. Additionally, the Contractor shall hire a Montana accredited asbestos inspector to perform a new asbestos survey on the building and any exterior elements proposed for demolition. Any new building materials found to contain asbestos, thereby requiring abatement, will be considered an unforeseen site condition which will require a contract modification. Provide report to Government with findings of survey.

2.1.6. The Contractor shall conduct a lead paint survey of all painted building materials proposed for demolition and provide a report of these findings to the Government.

2.1.7. The Contractor shall perform any site surveys needed to identify locations of utilities and grades prior to beginning demolition. Utilities shown on the drawings are approximate and must be verified by the Contractor.

2.1.8. Provide design to demolish all building components including foundation and footings, sidewalks, force protection wall and berm, trees and shrubs adjacent to the building and cut, remove and cap all building utilities. Utilities include water service, sewer service, storm, natural gas, power, HTHW and comm. See attached plans sheets to see extent of the demolition.

2.1.9. Provide design to backfill, compact and grade the site after demolition, install seed on the graded area, and install new irrigation system to cover the new seeded area. Repair any irrigation that is damaged during the demolition.

2.1.10. Provide as-built drawings of the site after work is complete.

2.2. Force Protection

2.2.1. Contractor personnel are required to abide by all real-world Force Protection Condition (FPCON) measures, random anti-terrorism measures (RAMs), shelter-in-place orders, evacuations and lock-down notices as directed. Examples include mandatory badge or ID checks, facility single points-of-entry and searches of hand carried items.

2.3. Anti-Terrorism (AT) Awareness Training

2.3.1. In accordance with DoDI O-2000.16v1_AFI 10-245-O, Antiterrorism (AT) Program Implementation, ensure Level I - AT Awareness Training is completed by all contractors within 30 days of requiring access, and annually thereafter. The training will be provided by the government.

The objective of the training is to provide personnel with requisite knowledge necessary to remain vigilant and enable employment of AT tactics, techniques, and procedures. The training will be conducted by 341st Contracting Squadron, by providing a written pamphlet for review.

2.4. Pictures on and DoD Facility

2.4.1. No pictures shall be allowed of any government property or component, which includes pictures of government property, from non-government property (EX: pictures of buildings physically on Malmstrom AFB, from exterior private property adjacent base perimeter), without prior consent from Malmstrom 341st Contracting Squadron. Any photograph, sketch, picture, drawing, map, or graphical representation of US Government property to include: facilities, aircraft, components, equipment, or other resources, is prohibited without first obtaining permission from the government (e.g. Security Forces convoys, Helicopters, Entry Control Points, etc.)

2.5. Criteria

2.5.1. The project will conform to National codes and Regulations for building construction and safety in addition to Military Handbook 1190 and Unified Facilities Criteria 3-600-01. The work shall conform to the current editions of Air Force Global Strike Command’s and Malmstrom’s Facilities Excellence Guidelines where applicable to this design. Modern construction techniques should be incorporated in the project to obtain both quality and economy and to provide a functional, complete, and usable system. Any document referenced by the Contractor for inspection will be included as a required submittal for the Contractor, if such documents are not currently available from Malmstrom AFB Civil Engineering.

2.6. References

2.6.1. Air Force Publications and Guide Specifications will not be furnished. They may be acquired from the Government Printing Office or other federal agencies. The Contractor shall specify products utilizing recycled material where feasible.

2.7. Government Furnished Information and Materials

2.7.1. The following information and materials will be furnished to the Contractor for the execution of the project work:

2.7.1.1. Existing Site, Building Plans, and Details if they exist. It is the responsibility of the Contractor to ask for these items if required.

2.7.1.2. Utility Maps, electrical, sewer, water, HTHW, and gas of the project site as available for use as reference.

2.7.1.3. Standard USAF title blocks and signature coordination to be used for final Full-Size drawings on CD ROM

2.7.1.4. Submittal schedule format for use with MS Excel (CD ROM)

2.7.1.5. Specifications in Microsoft Word (Electronically) for Contractor edits

2.7.1.6. Asbestos survey reports from 1995, 1997 and 2005

2.8. Pre-Design Conference

2.8.1. The pre-design conference between Contractor and Government representatives involved with the project will be held to ascertain that the Contractor clearly comprehends and understands the various scopes of work to be performed under each type of service. During the conference, the Contractor may present his/her views as to how he/she proposes to accomplish the project and to review and discuss the program requirements. A site visit will be conducted following the design conference.

2.9. Instructions and Administrations

2.9.1. During the course of project preparation, the Contractor may be called upon to attend conferences pertaining to the project. The Contractor shall prepare written minutes of all conferences, pertinent instructions and information received verbally or otherwise. Copies of minutes shall be transmitted to the attendees within 5 working days. Data submittals shall be by dated letter, and when necessary, hand carried. All documents will be on recycled paper copied both sides.

2.10. Visits

2.10.1. The Contractor or his/her duly authorized representative’s will be required to perform an investigation of the project site and may be required to make periodic visits to the project site as part of his contract, as necessary, to ensure design accuracy and timely completion of the project.

2.10.2. The Contractor and his duly authorized representatives shall comply with pertinent Air Force security regulations while working on the project. Special security clearances such as confidential, secret, etc., are not required by the Contractor for the design of this project. Take all possible precautions and efforts to prevent the disclosure of any information that might provide prospective construction contractors, manufacturers or suppliers as bidding advantage over others.

2.11. Scope

2.11.1 Prior to negotiations a meeting will be conducted to establish a mutual understanding of the project’s scope and contents.

2.12. Quality Control

2.12.1. The Contractor is responsible for the accuracy and content of plans and specifications.

Government review does not relieve the Contractor from the responsibility of conducting thorough in-house reviews prior to submitting to the Government. With each submittal, the names of each reviewer along with the particular areas reviewed will be submitted by separate letter entitled Certification of Quality Control. The letter will also address the total number of hours spent by these individuals in the review process.

2.13 Surveys

2.13.1. All surveying, where required, shall be the responsibility of the Contractor. The Contractor will be required to submit a copy of field notes and survey calculations as part of the design. Survey book identification numbers will be included on drawings.

2.14. Period of Service

2.14.1. Reference DD1155, Section F, Delivery or Performance and the project schedule.

2.15. Scope of Work

2.15.1. The Contractor shall perform all services required to prepare and furnish complete construction documents consisting of contract drawings, technical specifications, design analysis, cost estimate breakdown, and agenda as required for the accomplishment of the project in accordance with requirements of this specification.

2.16. Contract Drawings:

2.16.1. All contract drawings shall be prepared in accordance with the best professional practice.

Notes on the drawings should be held to a minimum. Extensive explanatory notes on the drawings are not desirable and should be placed in the specifications. The use of fractions less than 1/8 inch for building dimensions should be avoided. Use standard building material indications and symbols for architectural items and for mechanical and electrical equipment. The Contractor shall prepare and furnish original drawings to clearly indicate the extent, type, and detailed requirements for all related work to be performed. Drawings shall clearly indicate all new construction and delineate between all new work and existing conditions.

2.16.2. Drawings shall cover each phase of Architectural, Civil, Structural, Mechanical and Electrical categories and such other categories of work as required and or as accepted good engineering practice to provide a complete design product. Repair and alteration work items on drawings shall be specifically identified to delineate the work. Original drawings shall be prepared on 24” x 36” sheets, with the cover sheet on Mylar, 5 mil minimum, and the remaining sheets on paper. All drawings shall be prepared such that half size drawings are clear, sharp, and legible. Border widths on the drawings shall be in accordance with instructions and sample copy furnished by the Government. An appropriate title block shall be prepared in accordance with the Air Force sample furnished. The Contractor shall include the full names of the designer and the reviewer and the firm(s) they represent in the appropriate blocks on each drawing. The only other reference to the Contractor organization will be the professional seal on each drawing. The Contractor will be allowed access to any presently existing drawings pertaining to the project; however, such drawings will be used for general information purposes only and must be verified prior to use.

2.17. Submittals

2.17.1. Design development submittals shall occur in the following sequence:

2.17.1.1. 35% Completion: Prepare the preliminary design phase to include the following:

A. This submittal is to be indexed and tabbed, cover sheet titled and dated, and each page shall be numbered.

B. Drawings: All drawings, mechanical, electrical, structural, architectural, etc., shall be 35% complete. This submittal shall consist of 2 full size and 2 half- size drawings. All drawing sheets shall be numbered, and the cover page titled and dated.

C. Specifications: Develop preliminary specifications based on Construction Specification

Institute, CSI format.

D. Two CD ROM disks of specifications, AutoCAD Release 2014 or compatible drawings (*.dwg), and schedule of drawings.

E. Conference: Attend review conference at Malmstrom AFB to discuss design status and progress.

2.17.1.2. 85% Completion: Prepare the preliminary design phase to include the following:

A. This submittal is to be indexed and tabbed, cover sheet titled and dated, and each page shall be numbered.

B. Drawings: All drawings, mechanical, electrical, structural, architectural, etc., shall be 85% complete. This submittal shall consist of 2 full size and 2 half- size drawings. All drawing sheets shall be numbered, and the cover page titled and dated.

C. Specifications: Develop preliminary specifications based on Construction Specification Institute, CSI format.

D. Two CD ROM disks of specifications, AutoCAD Release 2014 or compatible drawings (*.dwg), and schedule of drawings.

E. Conference: Attend review conference at Malmstrom AFB to discuss design status and progress.

2.17.1.3. 100% Completion: Incorporate 85% review comments and prepare a package to include the drawings and specifications as follows:

F. Two full size sets and two half sized sets of final design drawings with Contractor professional seal.

G. One copy of the Preliminary DD Form 1354 (Transfer and Acceptance of Military Real Property).

H. One copy of list of design items which Contractor felt should have been accomplished but were deleted for various reasons.

I. One copy of annotated 100% review comments explaining actions taken on each comment.

J. Two CD ROM discs of pdf drawings, specifications, submittal schedule, schedule of drawings, and annotated review comments. The CD ROM shall have all necessary accompanying files to support full and accurate printing of the drawing files. Contractor shall verify in writing that disks contain all required documents.

2.18. Time Schedule

2.18.1. The Design and Construction Time Schedule shall be 190 calendar days from the issuance of the Notice to Proceed.

2.19. Schedule:

2.19.1 A pre-design conference will be held within 10 days of the Contract award date.

2.19.2 Submit 35% design for review and approval.

2.19.2.1 Offerors can expect to receive authorization for the commencement of demolition after the Government’s review and acceptance of the 35% design.

2.19.3 Submit 85% design for review and approval.

2.19.4 Submit final documents after formal approval of 85% contracting submittal.

2.20. Submittal Reviews: The Contractor will not be responsible for the time used by the Government in reviewing submittals.

2.21. Submittal: Number of copies to be submitted to the Contracting Officer:

2.22. Conferences Meetings

2.22.1. In addition to normal design review conferences, coordination conferences may be held from time to time as required. The Contractor or Government may request such conferences when it is deemed necessary by both parties to clarify the work or expedite the preparation of plans and specifications.

2.22.2. All review conferences and presentations will be at Malmstrom AFB.

2.23. Construction Closeout: The construction closeout documents shall consist of the following:

2.23.1. As-Built Field Data. The as-built field data shall reflect the exact installation performed and two copies shall be submitted to the Contracting Officer for review and approval a minimum of three calendar days prior to the date of final inspection. If review of the preliminary as-built drawings reveals errors and/or omissions, the drawings will be returned to the Contractor for corrections. The Contractor shall make all corrections and return the drawings to the Contracting Officer within 10 calendar days of receipt. The “Red LINE” drawings shall be reviewed at the final inspection for completeness. Once As-Built Field Data have been reviewed and approved one copy will be returned to the contractor with an electronic copy of contract drawings. The electronic contract drawings will be updated with the field data by the contractor and submitted to the Contracting Officer.

2.23.2. Approved preliminary as-built drawings will be returned to the Contractor along with one set of the contract drawing original record tracings. These drawings are part of the permanent records of this project and the Contractor will be held responsible for their protection and safety until they are returned to the Contracting Officer. Any drawings damaged or lost by the Contractor shall be satisfactorily replaced in like medium, quality and size as the originals at the Contractor’s expense.

The drafting work shall be performed by certified Engineering Technicians and/or individuals with a minimum of five years drafting experience. The name and qualifications of these individuals shall be submitted in writing to the Contracting Officer for approval.

2.23.3. The final as-built submittal shall be completed and returned together with the approved preliminary as-built drawings to the Contracting Officer within 30 calendar days of the final inspection. The Contracting Officer will review all final as-built record drawings for accuracy and conformance to the drafting standards and other requirements contained herein. The submittal shall be returned to the Contractor if corrections are necessary. The Contractor shall make all corrections and shall return the submittal to the Contracting Officer within seven calendar days of receipt.

3.0. CHECKLIST:

3.1. Contractor shall complete the Property Inventory checklist and submit it to the Contracting Officer 10 days prior to final inspection. Prior to completing the Form 1354 checklist, the Contractor shall request assistance from the Resource Management Flight. The checklist will be delivered to the Contractor at the Preconstruction conference.
