MEMORANDUM FOR [Past Performance Reference Organization]
FROM: [Offeror’s Address and Point of Contact]
SUBJECT: Request for Past Performance Evaluation
1. You have been identified as a point of contact for a past performance evaluation of the firm listed on the attached questionnaire. The 509th Mission Support Group is in the process of competitively selecting sources for the Multiple Award Task Order Contract (MATOC).
Whiteman AFB, MO has a need for a MATOC to provide indefinite delivery/indefinite quantity (IDIQ) design-build construction efforts to support the 509 MSG/CE mission.
2. One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts or other work efforts, which reflect the offeror's ability to perform on the proposed effort. The Air Force depends on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts. The Air Force’s areas of interest in the offeror are summarized in the enclosed questionnaire.
