1. Your assistance is respectfully requested in providing information on the attached performance questionnaire. The importance of your opinion cannot be overstated. This survey measures the present/past performance of a prospective offeror for providing Preventative/Corrective Maintenance Services on High- Expansion Foam Systems, Associated Alarm Panels, and Fire Pumps at Fairchild AFB, WA.

2. INSTRUCTIONS FOR COMPLETION: Please fill this questionnaire in by hand or type, and please email the results to the Contracting Officer(s), Zachery W. Ray at zachery.ray.2@us.af.mil, and to the Contract Administrator, Jackson Woods at jackson.woods@us.af.mil . Email submissions are preferred.

RATING DEFINITION

5 Substantial Confidence

Based on the offeror’s performance record, the respondent has a high expectation that the offeror will successfully perform the required effort.

4 Satisfactory Confidence

Based on the offeror’s performance record, the respondent has an expectation that the offeror will successfully perform the required effort.

3 Limited Confidence Based on the offeror’s performance record, the respondent has a low expectation that the offeror will successfully perform the required effort.

2 No Confidence Based on the offeror’s performance record, the respondent has no expectation that the offeror will be able to successfully perform the required effort.

1 Unknown Confidence

No performance record is identifiable or the offeror’s performance record is so sparse that no confidence assessment rating can be reasonably assigned.

Place an “X” next to the appropriate number for each item on the questionnaire. If the contractor has no record of past performance relevant to a particular question, select “1” for “Unknown Confidence” and provide narrative justification.
