DEMOLITION MULTIPLE AWARD CONTRACT (DMAC)

PERFORMANCE WORK STATEMENT (PWS)

FOR

SEED PROJECT

DEMOLISH BUILDING 1710

FAIRCHILD AIR FORCE BASE, WASHINGTON

Project No.: GJKZ 10-0171 12 May 2011

1. DESCRIPTION. The contractor shall perform demolition services of Building 1710 and surrounding pavements at Fairchild AFB, WA. The contractor shall perform work In Accordance With (IAW) the basic DMAC contract and this PWS.

1.1. SCOPE OF WORK. The contractor shall abate, demolish, properly dispose of and restore site of Building 1710 and surrounding pavements. Work includes hazardous materials abatement IAW the DMAC basic PWS and the attached hazardous material abatement survey report (Appendix E). Demolition includes but is not limited to; all floor slabs, footings, concrete docks and asphalt parking and driveways as indicated in Appendix C. Site restoration shall be performed IAW the standards identified in the basic DMAC PWS.

1.2. UTILITIES.

1.2.1. Electrical Demolition. Building 1710, receives power from an underground service lateral.

This lateral originates at the wood pole located just east of the access road (see drawing Appendix B), and has a disconnect switch mounted on it. The riser receives power from 120/208 volt conductors at the top of the pole and provides power to an attached floodlight (See Appendix D).

1.2.1.2. Demolition Requirements. The contractor shall demolish service conductors between the building and the pole, the riser, the disconnect switch and the underground service conduit to a point five feet beyond the building foundation. The contractor shall leave the base of the riser flush with the ground. Disconnect the riser from the 120/208 volt pole top conductors and insulate and tape the conductor ends. Abandon any remaining service conduit to the pole The contractor shall not demolish the floodlight or the pole or pole guy. Coordinate all electrical pole work with the base Electrical Shop. .

1.2.1.2.1. The 120/208 volt pole top conductors are connected directly to the un-fused secondary of a service transformer. There is no over-current protection on the circuit. Coordinate the de-energizing of the pole top circuit with the Base Electrical Shop. Lockout/tag out procedures shall be used.

1.2.2. Propane Gas Service. The propane tank servicing Bldg 1710 has been disconnected and relocated leaving the concrete slab and gas line. The contractor shall remove the gas line between the propane tank and Building 1710 and the concrete slab.

1.2.3. Water Service. Water lines at Bldg. 1710 have previously been cut and capped at the main.

Abandon the PVC pipe water line in place between the cap and the building.

1.2.4. Sewer System. The sanitary sewer system at the site is a pressure mound sewer system with associated septic tank, sewer lines, and pump station. The contractor shall decommission the sewer system to include pumping the vaults, capping the lines, removing and disposing of the septic tank and pump. See Appendix B for workload data.

1.2.5. Air Conditioning Units. Check all air conditioning units for Freon. If the units contain Freon, the contractor shall reclaim the Freon and provide all required collection and disposal documentation to the Contracting Officer.

1.3. ABATEMENT.

1.3.1. Lead Paint. The contractor shall remove and dispose of lead paint as hazardous material.

Concrete or CMU painted with Lead-Based Paint (LBP) shall be separated and disposed of as normal waste and shall NOT BE RECYCLED. The quantity of concrete with LBP (identified in Appendix E) shall be specified in the abatement and disposal plans. Proof that materials are non-hazardous may be required at landfills.

1.3.2. Asbestos Containing Material. Asbestos containing material (ACM) shall require abatement and disposal as indicated in Appendix E. The contractor shall submit all required manifests and disposal receipts.

1.3.3. Other Hazardous Containing Materials. Other materials indentified in the Hazardous Materials Survey require abatement and disposal as indicated in the report. The contractor shall submit disposal receipts and manifests as required.

1.4. PAVEMENT REMOVAL. Remove all the paved parking, driveway areas and concrete docks, sidewalks and steps from the areas identified in Appendix C. Pavement is estimated to be between 3 and 5 inches thick. After removing pavement, the contractor shall leave a smooth, straight saw cut edge. The contractor shall provide at a minimum, a 4 inch thick and 2 foot wide crushed gravel base shoulder at the edge of the cut pavement.

1.5. SITE RESTORATION. Site restoration under this task order shall include the parking and driveway areas and areas disturbed by demolition. Remaining restoration shall be completed IAW the basic DMAC PWS.

2. SUBMITTALS. The contractor shall provide all applicable submittals per the DMAC basic PWS.

3. PERFORMANCE PERIOD. The period of performance for this task order is 120 calendar days. Notices to Proceed (NTP) will be issued as per paragraph 1.2.2. of the basic contract PWS. A limite d NTP will be issued to complete work associated with CLIN 0001-0002

4. LIQUIDATED DAMAGES: In accordance with contract clause 52.211-11, Liquidated Damages (LDs) will be assessed for failure to complete the three phases within the time specificied in the approved performance shedule.

• LDs for Phase I will be assessed in the amount of $188.00 per calendar day.

• LDs for Phase II will be assessed in the amount of $211.00 per calendar day

• LDs for Phase III will be assessed in the amount of $179.00 per calendar day
