The purpose of this amendment is to provide a revised Performance Work Statement (PWS) in response to the Questions and Answ ers arrising from the 17 June 2010 site visit for the DMAC seed project.

1. PURPOSE: The contractor shall perform services for demolition of building 1710 and surrounding pavements at Fairchild AFB, WA. This SOW outlines the major items of work to be accomplished, but it does not attempt to explain the details and/or method of execution of this task order. The contractor shall refer to the basic DMAC contract for all specific contract requirements. This document does not relieve the contractor of any liability regarding compliance with any federal, state, and local laws.
2. SCOPE OF WORK: The contractor shall abate, demolish and properly dispose of Building 1710 and surrounding pavements and restore the site. Work includes hazardous materials abatement in accordance with the DMAC performance work statement. A hazardous material abatement survey report will be furnished (Attachment E). This survey shall be used as a basis for the materials abatement plan. Demolition includes all floor slabs, footings, concrete docks and asphalt parking and drives near building 1710 as indicated on Attachment C. Restore site in compliance with the DMAC performance work statement.
3. MAJOR ITEMS OF WORK:
3.1 Abate, demolish, dispose of building 1710 and restore site.
3.2 Remove all floor slabs, footings, concrete docks and asphalt parking and drives near Building 1710 as shown on the Drawing in Attachment C.
3.3 Utilities: Remove and cap all utilities at the mains.
3.3.1 Electrical Demolition of Building 1710
3.3.1.1 Existing conditions: The bldg receives power from an underground service lateral. The lateral originates at the wood pole located just east of the access road. The pole has a disconnect switch mounted on it. The riser receives power from 120/208 volt conductors at the top of the pole. The 120/208 conductors also provides power to a floodlight. See attached photos.
3.3.1.2 Demolition requirements: Demolish the service conductors between the building and the pole. Demolish the underground service conduit to a point five feet beyond the building foundation. Abandon the rest of the service conduit to the pole. Demolish the riser on the pole. Leave the base of the riser flush with the ground. Demolish the disconnect switch, and the riser going up the pole. Do not demolish the floodlight. Do not demolish the pole or the pole guy. Disconnect the riser from the 120/208 volt pole top conductors. Insulate and tape the conductor ends. Coordinate all electrical work on the pole with the base Electrical Shop. All work shall be in accordance with the National Electrical Code.
3.3.1.2.1. Caution: The 120/208 volt pole top conductors are connected directly to the un-fused secondary of a service transformer. There is no over-current protection on the circuit. All work at the pole shall be performed by licensed Electricians. Coordinate the de-energizing of the pole top circuit with the Base Electrical Shop. Lockout/tag out procedures shall be used.
3.3.2 Propane Gas Service: The propane tank servicing the building has been disconnected and relocated leaving the slab and gas line. Remove the concrete slab and the gas line to Building 1710. Contractor is responsible for all costs to remove the gas line to Building 1710.
3.3.3 Water Service: The water line already been cut and capped at the main. Abandon the water line between the cap and the building. The water line is PVC pipe.
3.3.4 Sewer System: The sanitary sewer system for Building 1710 is a pressure mound sewer system with associated septic tank, sewer lines, and pump station. Decommission the sewer system, including final pumping of the vaults and capping the lines. The pumps shall be salvaged and returned to the Government. The volume of material requiring pumping from the vaults is unknown. Remove and dispose off-base the septic tank and pump station in accordance with State of Washington guidelines. Details of the sewer system are provided in Attachment B. The existing pressurized drain-field is to be left in place.
3.4 Air Conditioning Units: Check all air conditioning units for presence of Freon. The contractor shall reclaim the Freon and provide all required disposal documentation to the Contracting Officer or QAP.
3.5 Abatement: The contractor shall remove and dispose of lead paint, asbestos containing material, and other materials identified in the hazardous material survey (Attachment E) as hazardous material in accordance with all Federal, State, Local laws.
Note: Any concrete containing adhesive, mastic, sealant, tile or any other material will be rejected by landfills unless proof (test results) can be provided showing material is non- asbestos.
3.5.1 Lead Painted Concrete Removal. Lead painted concrete or CMU is to be separated and disposed of as normal waste NOT TO BE RECYCLED. The quantity of concrete with LBP (identified in the Hazardous Materials Survey) shall be specified in the abatement plan and disposal plan. Follow Attachment H and all Federal, State and local laws and regulations for lead paint removal.
3.5.2 Asbestos Containing Material: Asbestos containing material (ACM) is anticipated to be was identified in the Hazardous Materials Survey and shall require abatement and disposal as indicated in the report. Follow Attachment G and all Federal, State and local laws and regulations for ACM removal. Submit disposal receipts and manifests.
3.5.3 Other hazardous containing materials. Other materials indentified in the Hazardous Materials Survey require abatement and disposal as indicated in the report. Follow all Federal, State and local laws and regulations. Submit disposal receipts and manifests.
3.6. Pavement Removal: Remove all the paved parking, drive areas and concrete docks, sidewalks and steps within the work area as identified in Attachment C. The asphalt pavement is anticipated to vary between 3 and 5 inches thick. Provide a smooth, straight sawcut at the edge of the pavement to remain. Provide a minimum 4 inches thick and 2 feet wide crushed gravel base for a shoulder at the edge of the cut pavement.
3.7. Site Restoration: Site restoration shall include the parking and drive areas and any other areas disturbed by the demolition. Restoration requires backfilling, including 4” of top soil and hyrdroseeded with dry land seeding mix in accordance with basic contract.
Submit a dry-land seed mix meeting Base Design Standards for approval.
4. SUBMITTALS: The contractor shall provide all applicable submittals per the DMAC performance work statement.
5. PERFORMANCE PERIOD: The period of performance is 120 calendar days.
6. EXCLUSION PERIOD: Winter exclusion period, pertinent to physical work phase, is not applicable to this project.
