Summary of Exhibit Q, Requirements Traceability Matrix (RTM)
This document is a Requirements Traceability Matrix for RFO No. HHS0017021 issued by the Texas Health and Human Services Commission (HHSC). The contract requires the contractor to provide staffing services comprising 52 managed services staff positions across three service tiers: Enterprise Service Desk (ESD) IT Support Specialist III (22 positions), DFPS Service Desk IT Support Specialist III (10 positions), TIERS IT/IEE Support Specialist III (5 positions), Deskside Support Systems Administrator II (14 positions), and Deskside Services Team Lead (1 position). The transition phase extends up to 60 calendar days from contract execution, during which the contractor must assign all four key personnel staff (Project Manager, Transition Manager, Hiring Manager, and Invoicing Manager) within one business day and achieve full staffing of 52 managed services positions within 30 calendar days. All qualified candidates must be located within a 50-mile radius of 701 W. 51st Street, Austin, Texas 78751. The contractor must participate in weekly transition status meetings, submit weekly transition status reports, and obtain HHSC written approval of an operational readiness checklist prior to commencing the operations phase.
The operations phase imposes comprehensive performance requirements organized into six categories: staffing, onboarding and security, staff changes, reporting and meetings, invoice and timekeeping, and performance and compliance. Key performance measures include maintaining minimum 85 percent monthly staffing compliance, submitting qualified candidate packages within five business days of request, completing staff training within 45 calendar days of hire, maintaining critical role vacancies below 10 business days per calendar month, holding monthly metric meetings by the 15th of each month, submitting weekly timesheets by Friday at 12:00 PM Central Time, and submitting monthly invoices by the 8th calendar day of the following month. The contractor is subject to liquidated damages for performance failures, ranging from $100 per business day for late candidate package submissions to $200 per business day for staffing compliance violations and equipment return failures. Three service level occurrences within a rolling 12-month period require submission of a corrective action plan. The contractor must comply with HHSC-directed staff removals within one business day, return all state property within two business days of separation, provide DFPS agency fingerprint-based background checks for DFPS service desk staff at contractor expense, and assume full liability for repair or replacement of any state property lost or damaged.
