About this file
This is an Exceptions Form (Exhibit E) associated with RFO No. HHS0017021, a solicitation issued by a state health and human services agency. The form provides respondents with a structured template to formally document any exceptions, deviations, or proposed modifications to the terms, conditions, or provisions outlined in the solicitation document. The form includes columns for respondents to identify the specific solicitation document section being challenged, cite the solicitation language in question, explain the basis for the exception, propose alternative language, and indicate whether they wish to remain under consideration for contract award if their exception is denied. Completion of this exhibit is optional and required only if respondents intend to take exceptions to the solicitation requirements.
The form explicitly establishes that no exception or alternative provision will be incorporated into any resulting contract unless the System Agency expressly accepts and incorporates it in writing. This mechanism allows respondents to formally notify the procuring agency of any terms or conditions they cannot accept as written while preserving their eligibility for award. The form functions as a critical procedural document in the solicitation process, ensuring transparency and formal documentation of all proposed deviations from the solicitation's standard requirements.
