About this file

This is an exhibit document from the Arizona Department of Community Services (DCS) Office of Real Estate outlining contractor notification requirements regarding Asbestos Containing Building Materials (ACBM) for janitorial services contracts. The document serves as a mandatory acknowledgment form that contractor supervisors must sign, confirming their understanding that DCS facilities may contain ACBM or Presumed Asbestos Containing Materials (PACM) as documented in available asbestos surveys. The exhibit establishes that contractors and their employees are prohibited from disturbing identified asbestos materials, and any accidental disturbance or discovery of suspicious materials requires immediate work stoppage and notification to DCS Risk Management at (602) 542-5698. All activities conducted within DCS facilities must comply with applicable state and federal guidelines, including OSHA and EPA regulations (29 CFR 1910.1001, 29 CFR 1926.1101, and 40 CFR 763.92).

The form requires contractor signatures and dates, along with verification that worker asbestos awareness training documentation has been completed and reviewed before work commences. This exhibit represents a critical health and safety requirement integrated into the janitorial services solicitation for the Payson area, ensuring that all contract personnel are informed of and acknowledge their responsibilities regarding asbestos-containing materials in state facilities. No pricing, award dates, or contract term information is contained within this exhibit, as it functions solely as a safety compliance and acknowledgment document that would be executed as part of the broader janitorial services contract award process.
