About this file
This is a Family and Recovery Environment Assessment (FREA) form used by the Department of Public Safety & Correctional Services, Department of Substance Use Disorder Services, as part of Solicitation #BPM055260 for pre-trial substance use disorder treatment services (OPS.XXX-XXX, Rev. 01/2026). The FREA is a brief assessment tool administered either through self-completion or clinician administration to evaluate how family history, current environment, and support systems impact an individual's recovery and reentry process. The form contains 17 yes/no questions organized across three sections: Family Impact and History, Current Family Environment, and Support and Education. Responses are intended to inform appropriate service and support recommendations for individuals in the substance use disorder treatment program.
The assessment gathers critical information regarding family involvement in substance use, history of abuse within family relationships, current family contact during incarceration, safety considerations for family participation in treatment, and individual preferences for family therapy, sober housing options, and educational support regarding addiction's impact on families. The tool also identifies potential barriers to recovery such as active substance use in the individual's intended post-release living environment and family conflict concerns. The form requires completion by the client or clinician, with signature and date fields for documentation purposes. This assessment serves as a clinical instrument to support individualized treatment planning and service coordination for individuals participating in the pre-trial substance use disorder treatment program.
