About this file

This document is a comprehensive Total Health Insight report prepared by Cigna Healthcare for the City of Miami, covering healthcare and pharmacy utilization, costs, and trends for the period from January 2024 to December 2024. The report provides a detailed analysis of the city's employee healthcare plan, which covers 4,195 members with an average employee age of 37.6 years, and includes a breakdown of medical and pharmacy spending, specialty drug trends, utilization management, and population health metrics. The document examines overall plan spend, which increased by 9.5% to $948.19 per member per month (PMPM), with medical spend rising 4.9% to $685.65 PMPM and pharmacy spend increasing 23.8% to $262.54 PMPM.

Key insights from the report include a significant rise in specialty pharmacy costs, particularly for drugs like Mounjaro and Ozempic, which increased to over $2.5 million in total cost. The analysis reveals 72 catastrophic claimants with costs exceeding $100,000 each, averaging $222,000 per individual and representing 35% of total costs. The report also highlights various cost management strategies, such as the SaveOnSP specialty copay assistance program, which could potentially save $530,535 or $10.54 PMPM, representing a 7.8% reduction in specialty pharmacy spend. Additionally, the document outlines health engagement programs, chronic condition management, and utilization management opportunities aimed at controlling healthcare costs while maintaining member health and wellness.
