About this file
This document is a Stop Loss Insurance Questionnaire for Request for Proposal (RFP) No. 2130386 issued by the City of Miami. The questionnaire is designed to gather comprehensive information from potential insurance providers for stop-loss insurance coverage. The document is structured into eight sections covering financial stability, contract terms, reporting capabilities, coverage details, underwriting and premiums, value-added services, legal compliance, and support and dispute resolution. Proposers are strongly encouraged to complete the questionnaire, as incomplete responses may negatively impact their evaluation score.
The questionnaire requests detailed information on proposed insurance costs, including specific and aggregate coverage options, monthly premiums, and alternative stop-loss levels. Proposers must provide their current financial ratings, company background, affiliated companies, and experience with public sector clients. The City seeks transparency on contract terms, claim handling processes, reporting capabilities, risk management services, and compliance with federal and Florida-specific healthcare regulations. The document includes spaces for proposers to outline their pricing structure, with specific requests for monthly costs for specific composite and aggregate coverage, as well as attachment factors for employees and families.
