About this file
This document is a City of Miami Questionnaire for Request for Proposal (RFP) No. 2130386, specifically for Flexible Spending Accounts (FSA) services. The questionnaire is designed to evaluate potential proposers for administering FSA and Dependent Care FSA (DCFSA) programs for the City of Miami, with the goal of selecting a provider for a five-year term with two additional three-year renewal options. Proposers are strongly encouraged to complete the questionnaire, as incomplete responses may negatively impact their evaluation score.
The document outlines comprehensive requirements across five key areas: Financial Stability and Experience, Plan Design and Services, Reporting and Recordkeeping, Technology and Integration, and Engagement and Support. The City seeks a provider capable of offering standard FSA and DCFSA services, including debit card issuance, IRS compliance, enrollment materials, customer service, employee meeting participation, and detailed reporting. The questionnaire requests information on the proposer's current ratings, experience with public sector clients, total FSA participants, reporting capabilities, technological integration, and willingness to support ongoing employee engagement initiatives.
