About this file
This is a Disclosure of Lobbying Activities Form SF-LLL required by the United States Department of Agriculture (USDA) in connection with the Hillsborough County Public Schools' Fresh Fruit and Vegetable Program procurement. The form is a standardized federal disclosure document used to report any lobbying activities undertaken to influence the outcome of covered federal actions, including contracts, grants, cooperative agreements, and loans. Vendors and contractors responding to this solicitation must complete and submit this form to certify whether they have engaged in lobbying activities related to the federal action. The form requires identification of the type of federal action, its status, the reporting entity, the federal department or agency involved, the federal program name, the federal action number, the award amount, and the names and addresses of any lobbying registrants or individuals performing lobbying services on behalf of the reporting entity. All responses must be submitted with signatures from authorized certifying officials along with their printed names, titles, and telephone numbers.
Completion of the SF-LLL form is mandatory for any organization seeking payment or agreement to make payment to lobbying entities for influencing federal officers, members of Congress, or congressional staff in connection with the covered federal action. The form must be filed at the initiation or receipt of a covered federal action or upon any material change to a previous filing pursuant to 31 U.S.C. section 1352. Failure to file the required disclosure subjects the reporting entity to civil penalties ranging from $10,000 to $100,000 for each violation. The completed disclosure is a material representation of fact upon which the federal government relies, and all information will be reported to Congress semi-annually and made available for public inspection. The form estimates a public reporting burden of approximately 10 minutes per response and is approved under OMB Control Number 0348-0046.
