About this file

This file is a Past Performance Questionnaire template designed for evaluating contractor performance on the Department of Veterans Affairs Wheelchair Van Transportation Service solicitation (36C24426R0021) for the Altoona VAMC. The questionnaire is completed by references familiar with an offeror's prior work and submitted to the Contracting Officer (cynthia.laemmelhirt@va.gov) by the RFP deadline to inform source selection decisions.

The questionnaire requires evaluators to assess relevancy of past contracts to the current VA solicitation using four tiers—Very Relevant, Relevant, Somewhat Relevant, or Not Relevant—based on scope and complexity alignment. Evaluators then rate contractor performance across 14 specific criteria using a five-point confidence scale (Substantial Confidence, Satisfactory Confidence, Limited Confidence, No Confidence, or Unknown Confidence). Assessment areas include compliance with contract requirements, timeliness, labor and safety standards adherence, management effectiveness, peak capacity response capability, communication systems and dispatch functionality, handling of scheduled and unscheduled veteran patient transports including evening and weekend services, complaint and accident reporting processes, staff integrity management, responsiveness to corrective action, administrative reporting timeliness, personnel management practices, and overall work quality. The form includes sections for contract-specific data (offeror name, contract number, customer agency, period of performance, total value, prime/sub/key personnel designation), point of contact information, detailed performance ratings with optional remarks, and evaluator identification. References with limited or no confidence ratings must provide explanatory comments, and contracting officers reserve the right to contact evaluators to verify responses.
