About this file

This is a Contractor Affidavit form regarding the use of coercion for labor and services, required by the School Board of Manatee County, Florida, for the Construction Management at Risk Tillman Elementary Campus Wide Renovation project (SDMC No. 2025-12). The affidavit must be completed and signed by an authorized representative of any nongovernmental entity executing, renewing, or extending a contract with the school board. The document requires the contractor to certify compliance with Florida Statutes Section 787.06(13) and to provide information including the contractor's name, address, phone number, authorized representative's name and title, and email address. The form serves as a sworn statement under penalty of perjury confirming that the contractor does not engage in prohibited labor practices.

The affidavit requires contractors to certify that they do not use or threaten physical force against persons; restrain, isolate, or confine individuals without lawful authority; establish exploitative debt arrangements; destroy or withhold government identification documents; cause financial harm; use fraud or deceit to lure persons; or provide controlled substances for exploitation purposes. This certification is mandatory for all vendors doing business with the School Board of Manatee County and must be signed by an authorized official of the contracting entity. The affidavit is a compliance document with no pricing, set-asides, or financial terms associated with it, serving solely as a prerequisite verification of the contractor's lawful labor practices prior to contract award.
