About this file
This is a Bidder's Statement of Principal Place of Business form required by the State of Florida for a Construction Management at Risk project involving a campus-wide renovation of Tillman Elementary. The form applies to bidders whose principal place of business is located outside the State of Florida and must be submitted as part of written bid, proposal, or reply documents in accordance with Section 287.084(2), Florida Statutes.
The form requires out-of-state bidders to provide a legal opinion from an attorney licensed to practice law in the bidder's home state regarding any state and political subdivision bidding preferences. Specifically, the bidder's attorney must certify whether the bidder's home state or political subdivision grants any preferences in the letting of public contracts to business entities with principal places of business within that jurisdiction. If preferences exist, the attorney must describe those preferences and identify the applicable state or political subdivision laws. The form requires completion by the out-of-state bidder's attorney, including signature, printed name, business address, telephone number, email address, and state bar admission information.
