About this file
This is a Contractor Affidavit document required under O.C.G.A. 13-10-91(b)(1) for contracts with Henry County, Georgia. The affidavit pertains to a bid for AV Equipment and Installation services at multiple schools within the jurisdiction. A pre-bid meeting was scheduled for March 1, 2023 at 11:30 AM Eastern Time via Zoom, with Raymond Braziel as the point of contact. Sealed bids were due to the Henry County Purchasing Department, located at 140 Henry Parkway, Room 209, McDonough, GA 30253 by 3:00 PM local time on March 21, 2023. Bids could not be withdrawn for sixty days after the submission deadline, except as permitted by OCGA. Henry County reserves the right to reject any and all proposals and to waive any technicalities.
The affidavit requires the contractor to register with and use the federal E-Verify work authorization program throughout the contract period, including any renewals or extensions. The contractor must notify the public employer if it ceases to use E-Verify, with such cessation constituting a material breach of contract. Additionally, the contractor must ensure all subcontractors submit compliant affidavits and must provide copies of required affidavits, drivers' licenses, and identification cards to the public employer within five business days of receipt. The affidavit must be notarized and incorporated into any resulting contract without further action or consent by the contractor.
