About this file

This is a Conflict of Interest Questionnaire (CIQ) disclosure requirement document issued by Dallas Independent School District in Texas, governing vendor disclosure obligations under Texas Local Government Code Chapter 176. The document outlines mandatory conflict of interest disclosure requirements for vendors seeking to conduct business with the District, effective for a Request for Proposal (RFP) to purchase Academic Enrichment and Support/Visual and Performing Arts instructional materials, goods, and services. The RFP is District-wide on an "as needed" basis and replaces RFP 206683, with vendors previously awarded on that contract encouraged to respond. The resulting contract shall have an initial term of three (3) years with two (2) successive one (1) year renewal options available at the District's discretion, not to exceed five (5) years total, contingent upon mutual agreement and long-range District needs. Either party must provide ninety (90) days' written notice prior to contract expiration if renewal is not intended.

Vendors must disclose conflicts of interest if they have employment or business relationships with any of the nine listed District board members or the Superintendent that result in taxable income exceeding $2,500 in the preceding twelve-month period, or if they have given gifts with an aggregate value exceeding $100 to such officers or family members within the same period. The completed CIQ form must be filed with the Dallas ISD Procurement Services Department at 9400 North Central Expressway Suite 1510, Dallas, Texas 75231, no later than the seventh business day after the vendor submits a bid or begins contract discussions. If no conflict exists, vendors must complete Box 1, type "N/A" in Box 3, and sign and date the form. Failure to comply with disclosure requirements constitutes a misdemeanor offense under Texas law.
