About this file
This document is Form 1295 guidance issued by Dallas Independent School District (DISD) in Texas for completion during the procurement process for Academic Enrichment and Support/Visual and Performing Arts Instructional Materials. The RFP seeks vendors to supply instructional materials, goods, and services supporting TEKS-based visual and performing arts instruction across multiple disciplines including band, choir, orchestra, piano, modern band, jazz band, mariachi, dance, elementary music, visual arts, theatre, and technical theatre. The procurement is designated for district-wide use on an as-needed basis, and existing vendors from the predecessor RFP 206683 are encouraged to respond. The contract term consists of an initial three-year agreement with two optional successive one-year extensions, not to exceed five years total. Either party must provide ninety days' written notice prior to contract expiration if renewal is not intended.
This procurement is issued in accordance with Title 2 of the Code of Federal Regulations Part 200 requirements and may utilize local, state, federal, and grant-funded program dollars. The Form 1295 guidance document provides instructions for completing the Certificate of Interested Parties disclosure as part of the proposal submission. All vendors are encouraged to participate regardless of whether they operate as sole source providers. Upon mutual agreement between the awarded vendor and the District, contract extensions may be granted not to exceed a cumulative five-year period.
