About this file
This is a coercion affidavit form required by the State of Florida for the Polk County School Board's armored car services solicitation. Effective July 1, 2024, Florida Statute 787.06(14) mandates that all nongovernmental entities executing, renewing, or extending contracts with governmental entities must provide a signed affidavit under penalty of perjury certifying compliance with labor coercion prohibitions. The affidavit requires the vendor's authorized representative to attest that the organization does not engage in physical force, unlawful restraint or confinement, exploitative debt practices, document confiscation, threats of financial harm, fraudulent enticement, or provision of controlled substances for exploitation purposes.
The affidavit must be completed with the vendor's name, address, phone number, and authorized representative information, then signed by the representative before a notary public. The notary must acknowledge the signature and provide commission details. This document serves as a mandatory compliance requirement for vendors bidding on the Polk County School Board's armored car and secure courier services contract, which encompasses transportation of deposits, negotiable instruments, confidential financial materials, and other authorized items between district locations and financial institutions.
