This is a Change Order (CO1) to Task Assignment 3 under Contract CN200 issued by the Florida Department of Environmental Protection (FDEP) to George F. Young, Inc., a professional engineering firm based in Gainesville, Florida. The change order amends the continuing professional services contract for the Gamble Rogers Memorial State Recreation Area project located in Flagler Beach, Florida, with Stuart Cullen, P.E., serving as the consultant contact person.
The change order authorizes three primary scope additions: task time extension, inclusion of previously designed but unbuilt West Entrance Area Improvements in the current plan set, and additional services for Coastal Construction Control Line (CCCL) permitting due to expanded scope. The fixed price cost increase is $17,859.12, with a reimbursable cost increase of $1,000.00 for a CCCL permit application fee, bringing the total change to $18,859.12. The original task amount of $32,037.56 increases to $50,896.68, and the task completion date extends from March 31, 2016, to July 29, 2016 (120 additional calendar days). The work comprises two primary subtasks: Subtask 4 covering FDEP CCCL permitting (topographic surveying, permit application, and responses to requests for additional information) totaling $15,944.94, and Subtask 5 covering 100% construction plans for the West Entrance Area Improvements totaling $2,914.18. All contract provisions apply unless specifically exempted, and the change order represents final agency action. The document was signed and executed on March 25-28, 2016, by authorized representatives from both FDEP and George F. Young, Inc.

Coastal Construction Control Line (CCCL) Permitting
West Entrance Area Improvements Design
Dear Brian:
Thank you for requesting George F. Young, Inc. to provide the attached additional services proposal summary and man-hour documentation for the above referenced project. As you are aware, the proposed design for the east side access has extended past the CCCL and permitting of this construction through the FDEP Beaches, Inlets, and Ports Department is necessary. We have also been requested to include the West Entrance Area
Improvements previously designed with the Campgrounds and Day Use Area project but not constructed in the project.
The attached proposal outlines the expected task work effort and deliverables. Please review this proposal and if acceptable, provide the appropriate paperwork to authorize us to proceed.
If you have any questions, please do not hesitate to contact me.
Sincerely, GEORGE F. YOUNG, INC.
