Deliverable: Survey deliverables will include an electronically signed and sealed topographic survey on 24x36 sheets in pdf format and .dwg CAD files. Paper copies can be provided upon request.

Performance Measure: Deliverables will be reviewed by the Department to confirm they are complete, accurate, and timely.

Fixed Price Cost: Man-hour Summary

Total for Subtask 1: $6,020.40

Firm Name Position Classification Hourly Rate Hours Total Fixed Price Cost

George F. Young Professional Surveyor & Mapper (Surveyor Manager) $105.00 5.00 $525.00 George F. Young Field Crew (2-Person) $70.59 60.00 $4,235.40 George F. Young CADD Technician $63.00 20.00 $1,260.00 Fee $6,020.40

Subtask 2: Geotechnical Engineering Description: We understand the subject site is relatively flat and easily accessible to truck mounted coring equipment. It is also our understanding and the basis of this proposal that traffic in the area is minimal such that the work may be completed safely exclusively with cones and signs with respect to traffic control.

The project site is located at the Silver Springs State Park in Silver Springs, Florida. The project consists of pavement improvements along NE 29th Place.

PSI proposes to perform the following services as part of the requested subsurface exploration program:

Underground Utility Clearance:

As required by law, PSI will contact “Sunshine One-Call” service to obtain underground public utility clearance. The underground utility clearance outside of the public domain is normally not addressed by the Sunshine One-Call system. PSI will not be held responsible for damages to private subsurface utilities.

If desired, PSI may obtain the services of a private utility locator, for an additional fee.

Subsurface Exploration PSI will perform a total of two pavement cores within the undisturbed roadway pavements adjacent to each reported location of distress (one at each location) followed by hand auger borings through the subgrade soils below and extended to depths up to 6 feet below the existing ground surface or to auger refusal, whichever is observed first. Upon encountering the roadway subgrade, the borings will be advanced by means of a manual post hole auger with samples collected and Dynamic Cone Penetrometer (DCP) test readings taken at regular 1-foot intervals until boring termination.

In addition, a total of four (4) hand auger borings will be performed along the Northeast 29th Place grassed right-of-way to depths up to 6-feet below the existing ground surface. Within each location, samples will be collected at regular 1-foot intervals until boring termination depth is reached or auger refusal conditions are encountered, whichever is observed first.

Encountered groundwater depths during drilling will be documented as part of the subsurface exploration work. The boring locations will be selected by estimating distances from features identified on the plans provided. The actual locations of the borings will ultimately depend on underground/overhead utility clearance and/or accessibility.

Upon completion of the borings the resulting boreholes will be backfilled with soil cuttings with any excess cuttings spread across the site.

Laboratory Testing Representative soil samples obtained during the field exploration program will be transported to our laboratory in Pensacola, Florida for visual classification and to perform a limited number of engineering properties tests. The nature and extent of the laboratory testing program will be dependent upon the subsurface conditions encountered during the field exploration program. Specific laboratory testing for this project will be determined once we have completed the field exploration phase of services, as well as the visual logging and classification of the samples obtained.

Reporting and Engineering Evaluations We will review the data from the field /laboratory to assess the impact of the prevailing subsoil conditions on the planned construction. The results of our work will be transmitted to you in a letter report that will include the factual data from the field and laboratory. The data will be reviewed by one of our geotechnical engineers with report prepared under the direction of one of PSI’s Chief Engineers or Principal Consultants. This report will include the following:

A plan of the site showing the approximate locations of the soil borings.

Logs of the exploratory borings, which will include the USCS soil classifications, brief soil descriptions and groundwater levels noted at the time of drilling. The profile logs will include measurement pavement asphalt and base-course material types and thicknesses where applicable.

General location and description of potentially deleterious materials encountered in the borings which may interfere with construction progress, including apparent existing fills or surficial organic materials. Recommendations for delineation of such materials will be provided, if applicable.

A brief geotechnical discussion related to the encountered shallow subsurface conditions revealed from our exploratory borings as they relate to the project.

The report will address site preparation options for dealing with the encountered subsurface conditions to allow for the construction of the new fill and concrete apron pavement. Electronic copies of the reports will be sent upon completion of our services. Up to two paper copies of the reports can be provided upon request.

Deliverable: The results of the field exploration and engineering analysis will be presented in electronically provided geotechnical engineering report in PDF format. A final report, signed and sealed by a professional engineer specializing in geotechnical engineering, will be provided.

Subtask 3: Limited NESHAP Asbestos Renovation Survey and Lead-Based Paint (LBP) Testing

Description 3: The testing is limited to materials/areas associated and accessible with the pipe replacement renovation project. It is also understood that limited areas consist of the roadway and drain areas but that the pipe which is underground and not accessible are not a part of this survey and may need to be tested once uncovered under a separate project.

PSI understands that the project consists of performing a limited NESHAP asbestos renovation survey and

LBP testing associated with the pipe replacement renovation project. All remaining structures within the

Silver Springs State Park are not a part of this project. This limited survey proposed by PSI is intended to meet the NESHAP and any Marion County Code requirements for limited renovation.

Reports shall be reviewed by a Florida Licensed Asbestos Consultant. The final summary reports are intended for the sole use of the client and no warrants or claims are made for use beyond that described herein. CAD drawings and photographs are not included as part of this survey. If the client provides PSI with a floor plan of the buildings, the inspector will place sample locations as a part of the survey.

Otherwise, the inspector will provide a field drawing with sample locations.

SCOPE OF LIMITED ASBESTOS SERVICES

An on-site survey of exposed and accessible building materials will be conducted by a team of U.S.

Environmental Protection Agency (EPA) Asbestos Hazard Emergency Response Act (AHERA) accredited inspectors. The inspectors will list materials found hat are suspected of containing asbestos and give a detailed description of their location. Any suspect materials found to be significantly damaged as defined by EPA will also be noted.

PSI will review all past reports, specifications, drawings, and other documents provided by the owner that would assist in identifying the suspect materials associated with the eight structures. This may also include interviewing maintenance personnel, State Park operations personnel or owners for their knowledge.

Limited Samples of building materials expected to be impacted/disturbed during the pipe replacement renovations will be collected according to EPA guidelines, which dictate the number and location of samples to be collected. PSI will attempt to sample damaged areas and areas that are least visible. This survey is limited to the suspect asbestos materials expected to be impacted in associated with the pipe replacement project. Please note, roofing materials will not be sampled as a part of this project.

It is anticipated that up to 30 asbestos samples (each sample collected includes laboratory analysis of up to two layers per sample; additional layers are considered an additional sample) will be collected and analyzed as a part of this project.

Patching and/or restoring of sample locations is not within the scope of work: PSI shall collect representative samples of each homogeneous area of suspect ACM by coring through the material to the base substrate. PSI shall collect samples as required by applicable regulations for the determination of asbestos content. Sample locations shall be randomly chosen to the extent possible; however, PSI shall preferentially collect samples from hidden or obscure locations. Additionally, PSI shall attempt to collect samples from areas of pre-existing damage.

The asbestos report will be reviewed by a Florida Licensed Asbestos Consultant. The final report is intended for the sole use of the client and no warrants or claims are made for use beyond that described herein.

Limited Lead-Based Paint Testing and Analytical Methodology

PSI will provide an experienced and EPA-certified Lead Inspector/Risk Assessor or trained PSI technician in the use of the X-ray Florescence (XRF) to conduct Lead-Based Paint testing. The Scope of the limited work for the Lead-Based Paint Inspection will include the use of an Heuresis, Pb200i, XRF Lead Paint

Analyzer to test only the building components to be impacted for the presence of lead-based paints. PSI understands that the client has requested only representative testing of areas to be impacted during renovations and that the HUD protocol is not a requirement for this project.

Our Inspector/Risk Assessor or trained PSI inspector in the use of the XRF will use an Heuresis, Pb200i

XRF Lead Paint Analyzer to determine if lead-based paints are present in association with the electrical/plumbing renovation areas to be impacted of the eight structures. The inspector will check the areas such as walls, ceilings, doors, door jams, windows, windowsills, floors, stairs, piping, ladders, railings, etc. As requested by the client, only measurements of random components will be tested from each interior and exterior painted surface expected to be impacted/disturbed during the electrical/plumbing renovations.

Client Responsibilities

It is the responsibility of the client to ensure the following in order to facilitate the efficient provision of the proposed services:

Construction documents, past survey reports, or other prior asbestos sampling data (if available).

As-built drawings of the facilities, prior to arriving at the site (if available).

An escort familiar with the property must be made available during the PSI inspection. In lieu of an escort, access to all areas of the structures, including those locations where mechanical system components are accessible, must be made available to PSI.

FDEP must provide safe and acceptable access to all areas of the building to be sampled. A revision to the professional fees proposed will apply if any areas of the Silver Springs State Park project are not accessible at the time of the survey and PSI is required to utilize an additional mobilization or wait to access areas.

Deliverable 3: Following completion of the field portion of the survey, one (1) electronic copy of the final report will be delivered to the client. This report will contain the detailed results of materials observed, sampling activities, and material quantification.
