About this file

This document is a Certification Regarding Lobbying form issued by the Nevada State Purchasing Division, Department of Administration, for use in state and federal contract solicitations. The form serves as a prerequisite certification that vendors must complete and submit when bidding on contracts involving federal funds or federal cooperative agreements. The certification requires the authorized representative of the vendor to attest that no federal appropriated funds have been or will be paid to influence federal officials, members of Congress, or their employees in connection with the awarding of federal contracts, grants, loans, or cooperative agreements. Additionally, vendors must disclose if any non-federal funds have been used for lobbying purposes by completing Standard Form-LLL in accordance with federal instructions.

The form requires vendors to include identical certification language in all subaward documents at all tiers, including subcontracts and sub-grants, ensuring that all sub-recipients certify and disclose accordingly. Submission of this certification is mandatory and constitutes a material representation of fact upon which the transaction is contingent, as imposed by section 1352, U.S. Code. Failure to file the required certification subjects violators to civil penalties ranging from $10,000 to $100,000 for each failure. The form contains blank fields for the date, solicitation number, vendor name, authorized signer name, and authorized signature, which must be completed by all bidders prior to submission.
