This document is a Questions and Answers (Q&A) file for a federal solicitation (75D301-25-R-00088) issued by the Centers for Disease Control and Prevention for a Survivor Clinical Center of Excellence (CCE) contract. The Q&A contains 17 questions from potential vendors, primarily Precision Healthcare Consultants and New York City Health & Hospitals Corporation, addressing topics such as small business subcontracting goals, proposal formatting, transition plans, and contract types.

Key details include CDC's small business prime contracting goals (11% Small Business, 3.22% Small Disadvantaged Business, 3% Women-Owned Small Business, 1.61% Service-Disabled Veteran Small Business, 1% HUBZone), a contract start date of July 1, 2025, and current services being performed under contract 75D301-22-C-15518 by New York City Health & Hospitals Corporation. The solicitation contemplates two contract types: cost-reimbursement only and cost-plus-fixed-fee, with a 40-page technical proposal limit and specific submission guidelines for electronic proposal transmission.

Is there any incumbent/recompete or this is a brand-new project? If yes, could you please provide us with their award number or even if the customer bought similar services in the past.

These services are currently being performed under the Centers for Disease Control and Prevention Contract 75D301-22-C-15518 by the New York City Health & Hospitals Corporation, which was one of multiple awards made against solicitation 75D301-22-C- 15518.

New York City Health & Hospitals Corporation.

Is there a preferred method of addressing each section of the PWS? i.e. can it be weaved into an overall narrative or is it required to have each item in the PWS listed out in the proposal.

Offerors should submit a proposal response that complies with and addresses all components of the solicitation requirements. It is up to the Offeror to decide on how it will develop a proposal that complies with and addresses all components of the solicitation.

New York City Health & Hospitals Corporation. Section L, pg. 3

Are there any formatting or organizational preferences for presenting the proposal and relevant information (i.e. file size limitations, attachments/appendices, font size) outside of the specifications of File Type and page limits?

Please refer to page 8 of Section L released via Amendment 001 on 4/10/2025. Please note that an Offeror's proposal should be in Times New Roman, 10 pt. font or larger.

Cover pages, table of contents, and appendices will not count towards the 40-page limit for the technical volume of an Offeror's proposal. Offerors should be mindful about the extent an Offeror makes use of appendices to substantially supplement its 40-page technical proposal.

New York City Health & Hospitals Corporation

Are there any alternative methods of transmission for these documents rather than individual email to the POCs listed? Large attachments are often flagged by both inbound and outbound email servers and may present a significant challenge in timely submission of all pieces of the proposal

All volumes and parts of the written proposal shall be submitted together at the same time by e-mail to the listed Contracing Officers. Volumes and parts of the written proposal may be submitted in multiple e-mails. The CDC e-mail client allows messages up to 100mb in total size and can accommodate single attachments between 75-80mb.

Offerors are encouraged to send follow-up e-mails to confirm the Government’s receipt of proposals. Offerors bear the responsibility to ensure their proposals are timely delivered to the proper location.

New York City Health & Hospitals Corporation Is there any anticipated change to the contract start date or performance period at this time?

No. There is no change to the anticipated period of performance start date of July 1, 2025.

New York City Health & Hospitals Corporation Section C-PWS Section 4.1, pg. 7-11 Transition In Plan: what is required of an incumbent in detailing the transition in plan?

The Government defers to Offerors to tailor their response according to the solicitation requirements. Please see Attachment Section C - Survivor CCE Performance Work Statement, page 7, Section 4.1.2 Transition-In for detailed requirements.

New York City Health & Hospitals Corporation

Section L, pg. 14; Section C-PWS Section 4.1

Can the Transition section of the PWS/Tasks to Be performed be addressed entirely within the Transition In/Out plan? Yes

New York City Health & Hospitals Corporation Section M, pg. 6

If transition in and transition out costs should be included in the business proposal as specified in Section M page 6, should they be indicated or designated in any way as potentially outside the timeframe of the contract (per FAR 52.237-3 Continuity of Services).

For purposes of estimating costs for the business proposal, transition-in costs should be captured during the base year and transition-out costs should be captured in the final year of performance (Option Period 4). Please clearly identify within the business proposal attachment the proposal costs that are attiritbutable to the estimated transition-in and transition-out timeline. The Government allows up to a 12-month transition period.

FAR 52.237-3, Continuity of Services is a separate extension from the transition-out period that is anticipated to occur prior to the end of the last option period (Option Period 4). Therefore, for estimating purposes FAR 52.237-3 should NOT be priced as part of the Offeror's business proposal. The Section L&M language as been revised accordingly.

New York City Health & Hospitals Corporation ATT J102; ATT J104

Cross Reference Matrix references 4 CLINs, but Budget spreadsheet has two CLINs: Travel and CCE Operations. What are columns F, G, H, and I meant to indicate in CRM?

An updated ATTJ102 has been provided; all tasks should be mapped to either CLIN 1 and CLIN 2. The template assumes that each task will be mapped to the same CLIN across all option years.

New York City Health & Hospitals Corporation Section L, pg. 11; Factor 1, A, b.

Operations Manual Summary pg. 11—what is this intended to look like, and what is the level of detail requested compared to the technical approach portion?

The Operations Manual Summary is the section of the technical approach where the Offeror is describing the specific processes and procedures that will be used to implement the overarching technical approach. The content should parallel the detailed requirements found in Attachment Section C - Survivor CCE Performance Work Statement, page 53, Section 8.3.4 Operations Manual, but be much less detailed. No specific format is required.

The Operations Manual Summary DOES COUNT towards the 40-page limit for the Offeror's technical volume proposal. The Operations Manual Summary requirement is meant to be a high level summary and it is up to the Offeror to determine the level of detail and extent of their technical volume proposal that will be dedicated to the Operations Manual Summary.

New York City Health & Hospitals Corporation Section L, pg. 20

Is it necessary that all projects and products be separated out via phase/option/segment within the business proposal format spreadsheet as specified on pg. 20 in Section L? Or only major activities that do not fall specifically under a PWS task?

Yes, costs need to be separated out such that the costs for each option year and the justification for the cost are all clearly documented. However, not every project/product anticipated within that year needs to be separated out; for example, printing/mailing member communications may be consolidated under one line item for each period.

However, each individual expense must be shown. The Offeror must still provide all working formulas and algorithms within the Business Proposal Format attachment (Microsoft Excel or compatible format). Values within spreadsheets and worksheets that are based upon values or calculations in another spreadsheet or worksheet should be linked to the other spreadsheet or worksheet to facilitate understanding of how the values were calculated or where they were derived from. Linked spreadsheets should also be provided with the Offeror's Business Proposal Volume.

New York City Health & Hospitals Corporation Section L, pg. 1

Can the Program please define the two types of contracts that are being considered? "Cost Reimbusement-only, no fee/profit" and "Cost-Plus-Fixed-Fee."

Please refer to FAR subpart 16.3 - Cost-Reimbursement Contract for definitions for cost reimbursement only and cost-plus-fixed-fee contracts.

New York City Health & Hospitals Corporation Section L, pg. 1

Is "Cost Reimbursement only, no fee/profit" contract type different from the Contract Reimbursement type currently held by H+H as a result of Notice ID 75D301-22-R-72142? If so, what are the differences?

The cost-reimbursement contract contemplate by the solicitation is defined in FAR subpart 16.3 - Cost-Reimbursement Contract
