About this file
This is an Insurance Requirements document for a contract with the Nevada State Board of Dental Examiners' Lobbying Services procurement. The document establishes mandatory insurance coverage that the contractor and all subcontractors must procure and maintain throughout the performance of work and any applicable warranty periods. The contractor must obtain Commercial General Liability coverage with a general aggregate limit of $2,000,000 and per-occurrence limit of $1,000,000; Automobile Liability coverage with a combined single limit of $1,000,000 for owned, hired, and non-owned vehicles; Workers' Compensation with statutory limits and Employers' Liability coverage of $100,000 per accident and $500,000 policy limit; and Professional Liability (Errors and Omissions) coverage with limits of $1,000,000 per claim and $2,000,000 annual aggregate. All policies must name the State of Nevada as an additional insured, and Workers' Compensation policies must include a waiver of subrogation against the state.
The contractor's insurance must be primary and non-contributory with respect to other available coverage sources, and all policies must be placed with insurers licensed in Nevada and carrying an A.M. Best rating of not less than A-VII. The contractor must provide certificates of insurance on ACORD forms or equivalent prior to work commencement and maintain coverage throughout the contract term. Failure to maintain required insurance or provide timely renewal evidence constitutes material breach of contract. The contractor must provide thirty days' prior written notice of cancellation to the State (ten days for non-payment of premium), and all subcontractors must be included as additional insureds under the contractor's policies or provide separate certificates of insurance. Any modifications to these insurance requirements require approval from the State's Risk Management Division or Attorney General's Office.
