About this file
This is a Certification Regarding Lobbying form issued by the Nevada State Purchasing Division, Department of Administration, applicable to the Nevada State Board of Dental Examiners' solicitation for Lobbying Services. The document serves as a mandatory certification that bidders and vendors must complete and submit as a prerequisite for contract award. The form requires the authorized signer to certify that no Federal appropriated funds have been or will be paid to influence Federal agency officials, Members of Congress, or Congressional employees in connection with the Federal contract, grant, loan, or cooperative agreement.
The certification contains material representations of fact and includes provisions requiring vendors to disclose any non-Federal funds used for lobbying activities by completing Standard Form-LLL if applicable. Vendors must also ensure equivalent certification language is included in all sub award documents at all tiers, including subcontracts and subgrants. The form includes fields for the solicitation number, vendor name, authorized signer name, signature, and date. Non-compliance with the certification requirements carries significant penalties, including civil penalties ranging from $10,000 to $100,000 for each failure to file the required certification, as mandated by Section 1352 of the U.S. Code.
