About this file

This is an attachment to a Request for Proposal (RFP) solicitation issued by the Arizona Department of Emergency & Military Affairs (DEMA) for Emergency Management Training services. The document specifies insurance requirements that offerors must satisfy as part of their proposal submission. Offerors are required to submit a Letter of Insurability from their insurance company demonstrating that they currently possess or can obtain the minimum scope and limits of insurance as detailed in Addendum A of the Special Terms and Conditions. This letter must be clearly marked as "Attachment - Supplement Insurance" in the proposal. The solicitation, numbered BPM00006951, established a submission deadline of May 30, 2025, for qualified vendors seeking to provide comprehensive training services for emergency management professionals within Arizona's governmental framework.

Upon contract award, contractors must provide a Certificate of Insurance (such as ACORD forms) along with associated policy endorsements electronically prior to commencing any services or work under the contract. Contractors are also obligated to maintain and provide updated Certificates of Insurance electronically on an ongoing basis to ensure the State maintains current insurance documentation as required by the contract terms. The solicitation does not specify a particular budget range or contract award value, and no set-asides, incumbent contractors, or specific funding sources are identified in this attachment. This insurance requirement is a standard compliance measure intended to protect the State and ensure contractor financial stability and risk management capability throughout the contract performance period.
