About this file
This is a Drug-Free Workplace Preference Statement form required for bid submission to the City of South Daytona, Florida for the Melodie Park Pond Dock/Decking Construction Project. Sealed paper bids are being accepted at the City Manager's Office, City Hall, located at 1672 S. Ridgewood Avenue, South Daytona, Florida 32119, with an electronic submission option available through the City's e-Procurement Portal at https://procurement.opengov.com/portal/southdaytona. The bid deadline is 3:00 pm on Wednesday, May 27, 2026. The project scope includes design, permitting, fabrication, delivery, and installation of a complete deck/dock system surrounding a newly constructed pond, with all work including labor, materials, equipment, engineering, supervision, transportation, mobilization, and demobilization. The contractor must provide marine-grade lumber framing, WearDeck composite decking material, and stainless steel hardware, obtain all applicable permits, and be an active Florida State Certified Marine Contractor performing all work in-house. Site visits are encouraged prior to bid submission to verify existing conditions.
This form establishes a drug-free workplace preference applicable to tied bids of equal price, quality, and service. Bidding firms must certify compliance with six specific requirements: publishing a workplace drug prohibition statement to employees, educating employees about drug abuse dangers and available assistance programs, distributing the prohibition statement to all relevant employees, requiring employee notification of drug-related convictions within five days, imposing sanctions or requiring rehabilitation program participation for violations, and maintaining good faith efforts to sustain a drug-free workplace. An authorized firm representative must sign and have the certification notarized. Firms meeting these drug-free workplace program criteria receive preference in award decisions when bids are otherwise identical, with established tie-bid procedures applied if no vendors maintain such programs.
