About this file
This is a Performance Assessment Questionnaire template issued by the U.S. Army Contracting Command - Aberdeen Proving Ground to obtain past performance evaluations of offerors responding to RFP W15P7T-27-R-A001 for Systems Engineering and Technical Assistance (SETA) Support for Public Safety Communications (PSC).
The questionnaire requests responses from identified points of contact regarding contractor performance on same or similar federal, state, or local government or commercial contracts classified under NAICS 541330 (Engineering Services). Respondents are asked to rate contractors across five evaluation criteria: compliance of products, services, and deliverables to specification requirements; effectiveness of project management including subcontractor control; timeliness of performance; effectiveness in forecasting and controlling estimated costs (applicable to cost-reimbursement contracts); and commitment to customer satisfaction and business-like concern for customer interests. Each criterion uses a five-point rating scale ranging from Exceptional to Unsatisfactory, with required rationale for each response. Responses must be submitted to Contracting Officer Katia Lacoste and Contract Specialist Rosa Acevedo no later than 15 days after receipt or by 30 September 2026, whichever occurs first. The questionnaire template includes sections for contractor identification, offeror representative information, evaluation ratings with rationale, general comments, identification of other relevant past efforts, and respondent identification. Responses are treated as source selection information and protected from public disclosure, though summary ratings will be provided to offerors in accordance with FAR 15.503(b).
