Version 004, 29 October 2021

APPENDIX A – SURVIVABILITY, RELIABILITY & VERIFICATION

1.0 First Article Testing (FAT)

First Article Tests will be separately priced items according to the terms and requirements set forth in each Task Order (TO). No deviations will be accepted without a modification to the TO. FATs are performed once for each Operational Transit Cases (OTC*) that contains a V1+ or V2, V1+, V2 and V3 configuration item to verify that the CHS-6 items satisfy all survivability requirements and meet the product specifications. This first article will serve as the manufacturing standard. Additional items procured, upholding the same configuration management requirements as the first item tested, will not need to reaccomplish FAT. IAW FAR 15.404-1(g), the price for the FAT and the pricing for the production quantities shall not be materially unbalanced.

All OTCs* that contain a V1+ or V2, V1+, V2 or V3 items added to the CHS contract undergo FAT unless specified otherwise in the accepted TO. FAT consists of hardware-specific survivability and reliability verification (visual, mechanical and electrical). At least one unit of the particular item undergoes FAT. V1 items are not required to have a FAT performed. FAT may be performed at the contractor’s facility or subcontracted and shall be witnessed and subsequently certified by the Government.

The delivery schedule for the production quantity will be included in the terms and requirements of the individual TO.

Note: OTCs shall undergo testing when required by the Government as specified in the TO. For simplification purposes the abbreviation OTC* is used in this document to signify OTCs that require testing rather than those that do not require testing.

Verification is through analysis; either mathematically using failure rates of sub-components, empirically through analysis of test data acquired during reliability testing or through analysis of actual field reliability data. Reliability verification required for FAT only.

2.6 Verification of Requirements for OTCs

2.6.1 First Article Test (FAT) for OTC

FAT OTC items are verified against the requirements specified in paragraphs 2.4 and 2.5 above and the OTC hardware specific (visual, mechanical and electrical) requirements set forth in a particular OTC product specification. At least one unit of the particular OTC configuration undergoes FAT.

1.6.2 Production Acceptance Test (PAT) for OTC

PAT is conducted on a 100% basis for every OTC item that is delivered after the first article (or articles). The PAT for OTC consists of hardware specific (visual, mechanical and electrical) testing only.

3.0 Summary of Survivability Criteria

3.1 All V1+, V2, V3 and OTC* equipment are required to meet the requirements set forth in Table A-1 below and witnessed and subsequently certified by the Government. Verification of V1+, V2, V3 and OTC* is specified in Paragraph 2, above.

3.2 Operational Transit Cases (OTC) that require testing are designated OTC* in Table A-1 below. As specified in Paragraph 2.4.8 every OTC is required to undergo Transportability testing. OTC* testing will be performed as specified in the TO.
