About this file
Exhibit 5B is a Contractor Signature Page for the Maryland Motor Vehicle Administration's (MVA) Environmental Compliance Handbook for Contractors. The document is a mandatory acknowledgment form requiring contractors working at MVA facilities to confirm receipt, review, and understanding of the environmental compliance guidelines. Contractors are obligated to review the handbook with their employees and subcontractors, maintain a copy at the worksite, and ensure comprehensive compliance with its contents. The signed document must be returned to MVA's Environmental Manager for recordkeeping.
The form provides space for both primary contractors and subcontractors to complete their detailed information, including signature, date, printed name, title, firm name, address, and contact information. This signature page is part of the larger V-MUL26004C On-Call Facility Repair and Renovation contract, which is a Small Business Reserve Procurement with specific requirements such as a 29% Minority Business Enterprise (MBE) participation goal. The document underscores the importance of environmental compliance and serves as a formal acknowledgment of the contractor's commitment to following the established environmental protocols at MVA facilities.
