About this file
This file is an Attachment M form from the State of Maryland requiring bidders and offerors to document their current or prior state contract experience. The form serves as a template for solicitation responses and applies to all state contracting entities within Maryland. Bidders are required to list all contracts with the State of Maryland for which they are currently performing or have performed within the last five years. For each identified contract, bidders must provide the contracting entity name and address, a description of goods and services provided, the dollar value, contract term dates, and the name, title, phone number, and email address of the state employee who directly oversaw the contract work. Additionally, bidders must disclose whether any contract was terminated before the end of its original term or if any available renewal options were not exercised.
The information collected through this attachment is used by the Procurement Officer to evaluate bidder responsibility and to assess past performance as part of the overall experience and past performance evaluation criteria for the solicitation. The form includes designated spaces for documenting multiple state contracts, with fields for state contracting entity details, contact person information, contract identification numbers, contract values, performance dates, and detailed descriptions of goods or services provided. This submission requirement ensures that the state has access to verifiable performance history and references for evaluating contractor capability and reliability for the Commercial Doors and Window Replacement project at the Victor Cullen Center, a secure Department of Juvenile Services treatment facility in Sabillasville, Maryland.
