SECTION 1200

MEASUREMENT AND PAYMENT

Measurement of Work Methods of Measurement One of the following methods of measurement for determining final payment is DESIGNATED IN THE SCHEDULE OF ITEMS for each PAY ITEM (Section 01201):

a) Actual Quantities (AQ).

b) Design Quantities (DQ).

c) Lump Sum Quantities (LSQ).

Measurement Terms and Definitions

1. Acre (AC). 43,560 square feet. Make longitudinal and transverse measurements for area computations horizontally unless specified on the ground surface. Do not make deductions from the area computation for individual fixtures having an area of 500 square feet or less.

2. Actual Quantities (AQ). These quantities are determined from measurements of completed work.

3. Design quantity (DQ). The quantity to be paid is the quantity shown in the bid schedule. The design quantity will be adjusted for authorized changes that affect the quantity or for errors made in computing this quantity. If there is evidence that a quantity specified as a design quantity is incorrect, submit calculations, drawings, or other evidence indicating why the quantity is in error and request, in writing, that the quantity be adjusted. Design quantities will be adjusted only when there are errors in the original design of 15% or more. Changes in the number of units DESIGNATED IN THE SCHEDULE OF ITEMS may be authorized under any of the following conditions:

a) Changes in the work authorized by the Contracting Officer.

b) A determination by the Contracting Officer that errors exist in the original design that cause a PAY ITEM quantity to change by 15 percent or more.

c) A written request submitted to the Contracting Officer showing evidence of errors in the original design that cause the quantity of a PAY ITEM to change by 15 percent or more. The evidence must be verifiable and consist of calculations, drawings, or other data that show how the designed quantity is in error.

4. Cubic yard (CY).

a) Cubic yard in place. Measure solid volumes by a method approved by the Contracting Officer or by the average end area method as follows:

(a) Take cross-sections of the original ground and use with design or staked templates or take other comparable measurements to determine the end areas. Do not measure work outside of the established lines or slopes.

(b) If any portion of the work is acceptable but is not completed to the established lines and slopes, retake cross-sections or comparable measurements of that portion of the work. Deduct any quantity outside the designated or staked limits. Use these measurements to calculate new end areas.

(c) Compute the quantity using the average end areas multiplied by the horizontal distance along a centerline or reference lines between the end areas. Deduct any quantity outside the designed or staked limits.

b) Cubic yard in the hauling vehicle. Measure the cubic yard volume in the hauling vehicle using three-dimensional measurements at the point of delivery. Use vehicles bearing a legible identification mark with the body shaped so the actual contents may be readily and accurately determined. Before use, mutually agree in writing on the volume of material to be hauled by each vehicle. Vehicles carrying less than the agreed volume may be rejected or accepted at the reduced volume.

Level selected loads. If leveling reveals the vehicle was hauling less than the approved volume, reduce the quantity of all material received since the last leveled load by the same ratio as the current leveled load volume is to the agreed volume. Payment will not be made for material in excess of the agreed volume.

Material measured in the hauling vehicle may be weighed and converted to cubic yards for payment purposes if the conversion factors are mutually agreed to in writing.

c) Cubic yard in the structure. Measure according to the lines of the structure as shown on the plans except as altered by the Contracting Officer to fit field conditions. Make no deduction for the volume occupied by reinforcing steel, anchors, weep holes, piling, or pipes less than 8 inches in diameter.

d) Cubic yard by metering. Use an approved metering system.

5. Each (EA). One entire unit. The quantity is the actual number of units completed and accepted.

6. Hour. Measure the actual number of hours ordered by the Contracting Officer and performed by the contractor.

7. Linear foot. As applicable, measure the work along its length from end-to-end; parallel to the base or foundation; along the top; along the front face; or along the invert. Do not measure overlaps.

8. Mile. 5,280 linear feet. Measure horizontally along the centerline of each roadway, approach road, or ramp.

9. Lump sum quantity (LSQ). These quantities denote one complete unit of work as required by or described in the contract, including necessary materials, equipment, and labor to complete the job. Do not measure directly. The bid amount is complete payment for all work described.

10. Station. 100 linear feet. Measure horizontally along centerline or reference line of each roadway, approach road, or ramp.

Weighing Procedures and Devices

Batch masses may be acceptable for determination of pay quantities when an approved automatic weighing, cycling, and monitoring system is included as part of the batching equipment.

When a weighing device is determined to indicate less than true mass, no additional payment will be made for material previously weighed and recorded. When a weighing device is determined to indicate more than true mass, all material received after the last previously correct weighing accuracy test will be reduced by the percentage of error in excess of 0.5 percent.

When material is proportioned or measured and paid for by mass, provide one of the following:

(a) Commercial weighing system. Use permanently-installed and certified commercial scales.

(b) Invoices. If bulk material is shipped by truck or rail and is not passed through a mixing plant, furnish a supplier's invoice with net mass or volume converted to mass. Periodic check weighing may be required.

Receiving Procedures

When the method of measurement requires weighing or volume measurement in the hauling vehicle, furnish a person to direct the spreading and distribution of material and to record the location and placement of the material on the project. During the placement, maintain a record of each delivery and document it in an acceptable manner. Include the following information as applicable:

(a) Project identification;

(b) Contract pay item number and description;

(c) Location where placed;

(d) Date;

(e) Load number;

(f) Truck identification;

(g) Time of arrival;

(h) Mass or volume; and

(i) Spread person's signature.

Use an approved format for the delivery record(s). Furnish the original record(s) and a written certification of the delivery of the material at the end of each shift.

Scope of Payment

Payment for all contract work is provided, either directly or indirectly, under the pay items shown in the bid schedule.

1. Direct payment. Payment is provided directly under a pay item shown in the bid schedule when one of the following applies:

a. The work is measured in the Measurement Subsection of the Section ordering the work, and the bid schedule contains a pay item for the work from the Section ordering the work.

b. The Measurement Subsection, of the Section ordering the work, references another Section for measuring the work and the bid schedule contains a pay item for the work from the referenced Section.

2. Indirect payment. Work for which direct payment is not provided is a subsidiary obligation of the Contractor. Payment for such work is indirectly included under other pay items shown in the bid schedule. This includes instances when the Section ordering the work references another Section for performing the work and the work is not referenced in the Measurement Subsection of the Section ordering the work.

Compensation provided by the pay items included in the contract bid schedule is full payment for performing all contract work in a complete and acceptable manner. All risk, loss, damage, or expense arising out of the nature or prosecution of the work is included in the compensation provided by the contract pay items.

Work measured and paid for under one pay item will not be paid for under any other pay item.

The quantities shown in the bid schedule (Section 01201) are approximate unless designated as a design quantity. Limit pay quantities to the quantities staked, ordered, or otherwise authorized before performing the work. Payment will be made for the actual quantities of work performed and accepted or material furnished according to the contract. No payment will be made for work performed in excess of that staked, ordered, or otherwise authorized.

SECTION 01505

MOBILIZATION AND DEMOBILIZATION

GENERAL

MOBILIZATION

Mobilization shall consist of preparatory work and operations, including, but not limited to, those necessary for the movement of personnel, equipment, supplies, and incidentals to the site; for the establishment of all facilities necessary for work on the project; and for all other work and operations which must be performed, or costs incurred prior to beginning work, on the various items on the project site. Mobilization includes obtaining project-specific bonds and obtaining any permits, including the EPA’s Construction General Permit NOI (electronic NOI system, “eNOI”) for stormwater runoff, not provided by the Contracting Officer. Contractors shall note the 14 day waiting period between submittal to the eNOI system and breaking ground. The Contractor shall have erosion control devices on site, including, but not limited to, 1 6-inch pump, silt fence, stakes, waddles, and straw bales, in accordance with Section 02140 prior to any ground disturbance.

Mobilization shall also include the construction of temporary ramps and access ways, staging areas, grading, and the necessary preparatory work required to allow for the safe and stable movement of all vehicles that are required to construct the improvements outlined in the Contract Documents. Maintenance of the levee road and Road 233 is incidental to mobilization. An access management and staging plan including consideration of loads scheduled to cross the temporary bridge is required before starting work.

There is a temporary bridge in place between the South staging area and stockpiles and the project location. Contractor shall ensure no loads crossing this bridge exceed its capacity. The temp bridge has been rated as follows:

42 ton GVW for a type 3 truck axel configuration 66 ton GVW for a type 3S2 truck axel configuration 81 ton GVW for a type 3-3 truck axel configuration 44 ton GVW for a 336 Excavator 43 ton GVW for a D8 Caterpillar Komatsu HM300-2 articulated dump truck with a 53,000 lb vehicle weight, 40,300 lb payload, and a gross weight of 113,400 lb. This vehicle rated out at .88. It would be limited to 2/3 capacity load crossing the bridge.

No tracked vehicles shall operate on bridge without approved bridge deck protection. Contractor shall submit an overload permit for any GVW over 60 ton and GVW over 40 ton for a vehicle with less than 4 axels.

1. Notify the Contracting Officer in writing at least 72 hours before moving any construction equipment onto the National Forest. Notification will include an agreed upon location where the equipment will be available for inspection by the Nez Perce Tribe or Forest Service. Subsequent move-ins of equipment shall be treated the same as the initial move-in and will require notification.

2. All equipment shall be washed prior to mobilization to the Forest to minimize the introduction of foreign materials and fluids to the project site. All equipment shall be free of oil, hydraulic fluid, and diesel fuel leaks. To prevent invasion of noxious weeds or the spread of whirling disease spores, all equipment shall be power washed or cleaned to remove all dirt, plant parts and material prior to mobilization. Truck beds and dump boxes hauling to the project site shall also be cleaned prior to entering the Forest. It will be the contractor's responsibility to insure that adequate measures have been taken. Equipment shall be inspected by the Inspector prior to mobilizing to the Forest. Only construction equipment that has passed inspection by the Nez Perce Tribe or Forest Service will be allowed to operate within the project area.

3. Equipment shall be in a well-maintained condition to minimize the likelihood of a fluid leak. Any leaks or accumulations of grease would be corrected before entering the project area. If a fluid leak does occur, the project Inspector and COR shall be notified immediately, and all work ceased until the leak has been rectified. At all times during the construction phase, fluid spill containment equipment adequately sized for specific quantities of fuel shall be present on-site and ready for deployment should an accidental spill occur. Fuel spill kit shall include an oil-absorbing floating boom and absorbent pads. Contractor and Contractor’s representatives shall be knowledgeable on use of the spill containment equipment. The Contracting Officer reserves the right to refuse equipment that does not meet criteria.

4. Equipment that will be used in live water shall utilize hydraulic fluid that is non-toxic to salmonids. The following criteria should be met to determine if a hydraulic fluid is nontoxic to salmonids during acute exposure: (a) The test species used should be a salmonid (most often this will be rainbow trout, but occasionally Chinook salmon or Coho salmon are tested); (b) The test duration should be 96 hours; (c) The test should be conducted using the water accommodated fraction (WAF) (the WAF is used in testing hydrophobic materials to provide a "worst case scenario" for exposure to aquatic organisms); and (d) The value of the LC50 should be >1000 mg/L. Several products on the market meet these specifications.

According to the guidance provided by the National Oceanic and Atmospheric Administration, the following products meet the required criteria:

· Chevron Clarity Hydraulic Oil A W

· Ecoterra Hydraulic Oil

· GreesCare Plus

· Mobil EAL 224H

· ENVIRONTM AW Hydraulic Fluids These products and companies are not endorsed by the government and the contractor should perform independent research regarding the performance characteristics of each product.

5. All construction equipment shall be staged in a location and manner to minimize air, soil and water pollution. Storage of fuel and lubricants - all fuel and lubricants shall be stored in containers and areas that are in conformance with the Idaho State Department of Environmental Quality, NMFS and USFWS biological opinion terms and conditions, and local regulations.

6. Servicing and refueling equipment - all fuel and lubricants used in the servicing of construction equipment shall be done in a manner that avoids spills and over filling and shall be at least 150 feet from all waterbodies. Stationary power equipment, such as generators and pumps, within 150 feet of water shall be diapered to prevent leaks. All power equipment within 150 feet of the water shall be inspected daily for fluid leaks and repaired. The contractor must keep daily inspection reports in a diary.

7.The Idaho State Department of Environmental Quality shall be notified immediately of any spill and the operator shall contain the spillage. If a spill of chemical pollutants such as fuel or hydraulic fluid should occur, immediately attempt to contain the spilled material. The following procedures shall be followed:
(a)for spillage on land, construct earthen berms or use other suitable barricade material of sufficient size to contain the spell and keep it from spreading.

(b) for spillage on water, attempt to isolate and contain the spilled material. Commercial booms or other suitable materials shall be kept on site during construction to contain fuel and oil spills on water.

8. Sanitary facilities - sanitary facilities such as chemical toilets shall be located at least 150 feet from water bodies to prevent contamination of surface or subsurface water.

9. A description of hazardous materials that will be used including inventory, storage and handling procedures will be available on site.

10. If elemental mercury is found during project work, work in the immediate vicinity will stop and every reasonable effort will be made to contain the material in such a manner that it will not reach surface or groundwater. The COR shall be contacted immediately.

SITE PREPARATION

1. Existing Conditions The Contractor shall determine the actual condition of the site as it affects this portion of the work. Particularly, Contractor shall inspect the temporary haul road for erosion and repair as necessary for use. There is one location where the temp road fill was removed to allow for fall/spring drainage. The Contractor shall insert a temporary 18” minimum culvert and backfill. This fill and any culvert(s) shall be removed upon completion of Phase 2. Any fill and maintenance of the temp road shall be compacted with heavy equipment.

2. Protection of Existing Facilities and Improvements Site preparation shall not damage existing structures, landscaping, or vegetation adjacent to the areas designated for site preparation. The Contractor shall repair or replace any and all damaged property.

3. Vegetation Preservation and Salvage Preservation of existing vegetation and trees is of utmost importance. The Contractor will stake and walk the entire site with the Inspector and Contracting Officer. The Contractor may plant in the areas already preserved, however, utmost care shall be used to protect the exiting vegetation.

DEMOBILIZATION AND SITE RECLAMATION

1. Demobilization shall consist of work and operations necessary to disband all mobilized items and clean up the site. The removal of all temporary ramps, access ways, roads, signs, temporary fencing, construction debris including rock chips, wood debris, construction stakes, and other construction-related refuse, and temporary facilities or works and the restoration of surfaces to an equal or better than existing condition shall also be included as part of demobilization. No erosion control debris, including brush mats used to control erosion, may be left in the channel upon completion of work.

2. Site reclamation is included under demobilization. Site reclamation includes reclamation of areas disturbed during construction, including staging areas, and temporary access roads. Staging areas shall be decompacted and graded to match elevation specified on the plan sheets.

3. All damaged or disturbed streambanks are to be restored to a natural slope pattern and profile for establishment of permanent woody vegetation. Leave native materials, e.g., down wood, where they are found, the floodplain is at grade. If native materials (e.g., downed wood) are destroyed, replace them with a functional equivalent during site restoration.

4. Stockpiled materials (i.e. trees, vegetation, topsoil, and other excavated material from restoration project areas) salvaged under Phases 1-2, shall be used as specified in the plan sheets.

5. All brush mats, straw bales, and erosion control devices shall be removed from below the ordinary high water mark upon completion of the project. Small straw bales may be scattered upon the project site above the ordinary high water mark.

6. All scrap metal that was left at the south staging area shall be removed and disposed of by the Contractor.
