About this file
This is a Past Performance Questionnaire used to evaluate contractor suitability for the Library of Congress solicitation 030ADV26R0055 for Digital Talking Book Machine Battery Packs. The questionnaire serves as an evaluation tool for current or previous customers of bidding contractors to assess their performance on related projects. The evaluation criteria include overall contract management, product and service quality, project phase management, schedule compliance, report timeliness, subcontractor effectiveness, personnel competence and communication, cost performance, and contract requirement fulfillment. Respondents must provide narrative explanations for any Marginal, Unsatisfactory, or Not Applicable ratings and are instructed to return completed questionnaires directly to Leslie Medina (lmedina@loc.gov) and James Robinson (jarobinson@loc.gov) by the specified deadline at 5PM Eastern Time.
The questionnaire template requires respondents to provide general information including the evaluating company name, point of contact details, relevant contract number, contract type, period of performance, contract dollar value, and contract description.
