About this file

This is a Past Performance Questionnaire used by the Library of Congress to evaluate contractor suitability for solicitation 030ADV26R0055, which seeks bespoke batteries for Digital Talking Book Machines (DTBMs) used by National Library Service (NLS) patrons. The questionnaire is to be completed by current or previous customers of prospective contractors and returned to Leslie Medina (lmedina@loc.gov) and James Robinson (jarobinson@loc.gov) no later than October 27, 2026 at 5:00 PM Eastern Time. Responses may or may not be anonymous but are critical to the proposal evaluation process.

The questionnaire requests general company information including the contractor name, point of contact, telephone number, contract number, contract type, period of performance, dollar value, and contract description. Respondents then rate contractor performance across ten dimensions using a five-point scale (Outstanding, Good, Satisfactory, Marginal, Unsatisfactory) plus a Not Applicable option. Rating categories address overall contract management, product and service quality, project phase management, schedule compliance, report timeliness, subcontractor effectiveness, personnel competency, communication, cost performance, and any contract terminations or rescheduling due to performance issues. Narrative explanations are required for any Marginal, Unsatisfactory, or Not Applicable ratings, supplemented by judgments from other organizational members. This questionnaire is part of Amendment 2 to the solicitation, which modified the past performance evaluation requirements.
