About this file
Attachment J is a Foreign Country of Concern Attestation form issued by the Florida Department of Management Services for the Productivity Tools (RFP No. 25-43231513-RFP) solicitation. The form requires an officer or representative of an entity submitting a bid, proposal, or contract to affirm that their organization is not owned by, organized under the laws of, or controlled by the government of a Foreign Country of Concern. This attestation is specifically required for contracts that would grant the entity access to an individual's Personal Identifying Information.
The form is a standard compliance document that must be completed under penalties of perjury, with the entity representative providing their printed name, title, signature, and date of attestation. It is part of the broader procurement process for the statewide Productivity Tools Term Contract, which seeks vendors to provide digital productivity solutions for state agencies and eligible users across collaboration, communication, office productivity, cloud services, and workflow automation categories. The attestation helps ensure that vendors meet specific statutory requirements related to foreign ownership and control, which is part of the state's broader vendor eligibility and security screening process.
