About this file
Attachment G is a Facility and Equipment Checklist for the State of Florida Department of Management Services (DMS) RFP DMS-2526-098 for lawn care services across multiple regions. This checklist requires respondents to provide yes or no responses to eight inspection criteria covering equipment storage, maintenance, safety, and operational readiness. Respondents must confirm that equipment is stored in secure facilities protected from the elements, maintained in good condition according to manufacturer specifications with available service records, has all required safety measures in place, is fully functional, includes commonly replaced parts on hand, has properly labeled and stored fuel, and can service multiple facility groups simultaneously from a local facility. Failure to provide responses to any criterion may result in a vendor being declared non-responsive, and the Department reserves the right to declare a respondent not responsible based on negative responses.
The checklist serves as a preliminary qualification document to assist DMS in evaluating the capability and responsibility of respondents bidding on lawn care services contracts across the Pensacola, Tallahassee, and Tampa regions. Respondents must provide their facility addresses and be authorized representatives when submitting the completed checklist. The Department intends to award contracts to multiple respondents in each region but reserves discretion to award to a single respondent per facility group, make a single award for all regions, or make no award based on the state's best interest determination.
