About this file
This is a Past/Present Performance Questionnaire template (Attachment 6) for solicitation HTC71125RW001 related to Scheduled Fuel Barge Transportation in the Atlantic Region. The questionnaire is divided into four sections - Contract Identification (to be filled out by the offeror), Past/Present Performance Reference Information, Performance Information, and Narrative Description (to be completed by references).
The questionnaire collects detailed information about the offeror's previous contracts including contract type, value, period of performance, and specifics about fuel transportation services provided - including types of fuel moved (Jet-A, JP5, F76), monthly delivery volumes, delivery locations, and areas of operation (US Atlantic Coast, Gulf Coast, inland waterways, etc.). It evaluates technical capabilities across 10 criteria including fuel transportation performance, safety, timeliness, and operational reporting, using a rating scale from Unsatisfactory to Exceptional. References must also provide narrative responses about any marginal/unsatisfactory ratings, contract terminations, contractor strengths, and potential concerns. Completed questionnaires are to be submitted to the USTRANSCOM Acquisition Office.
