About this file

This is a Past Performance Questionnaire form that serves as Attachment 5 to a Federal Aviation Administration (FAA) acquisition under the SAVES HWSW Screening Information Request (SIR) 697DCK-25-R-00302. The questionnaire is designed to evaluate contractor performance on previous contracts to assess their suitability for the SAVES HWSW contract opportunity.

The four-page form requires completion by government personnel or previous clients who have worked with the contractor being evaluated. It includes sections for basic contract information (contractor name, contract numbers, values, performance periods), respondent details, and comprehensive performance ratings across five key areas: Quality of Service, Timeliness of Performance, Customer Satisfaction, Customer Service Relations and Effective Management of Assets, and Ability to Manage Cost. The rating scale ranges from Excellent to Unsatisfactory, with specific definitions for each level, and requires narrative explanations for any ratings below Satisfactory. The completed questionnaire must be returned to FAA Government Points of Contact Dawn Bloome and Kristin Frantz at their specified email addresses, though no specific deadline is provided in the visible portions of the form.
