About this file
This is a Past Performance Questionnaire template issued by the Federal Aviation Administration (FAA) for the SAVES HWSW Screening Information Request (SIR) 697DCK-25-R-00302. The questionnaire is designed to collect performance feedback from references regarding contractors bidding on or involved in FAA SAVES contracts.
The form requires respondents to evaluate contractor performance across five major categories: Quality of Service (8 questions), Timeliness of Performance (3 questions), Customer Satisfaction (4 yes/no questions), Customer Service Relations and Effective Management of Assets (6 questions), and Ability to Manage Cost (4 questions). Respondents use a five-point rating scale (Excellent, Good, Satisfactory, Marginal, Unsatisfactory, or N/A) to assess performance against contractual requirements. The questionnaire also collects administrative information including the contractor name, contract number, contract value, period of performance, respondent details, and length of involvement. Completed questionnaires must be returned to Contract Specialists Dawn Bloome and Kristin Frantz at the FAA by a specified deadline. A comments section is provided for narrative explanations of any ratings below "Satisfactory." Handwritten responses are acceptable.
