About this file
This file is a Contract Performance & Quality Assurance Report template used for cemetery maintenance and renovation projects. The document contains multiple standardized forms designed to inspect and document contractor compliance with statement of work requirements across four main service categories.
The first form addresses soil compaction testing for subgrade, aggregate base, and amended topsoil, requiring inspection frequencies (daily, weekly, or random) with specific testing density standards: 90% maximum dry density for subgrade (1 test per 2,000 sq-ft, minimum 6 tests), 95% maximum dry density for aggregate base (1 test per 2,000 sq-ft, minimum 6 tests), and 85% maximum dry density for amended topsoil (1 test per 20,000 sq-ft, minimum 6 tests). The second form covers upright headstone reset, realignment, backfill, and cleaning operations, requiring eight compliance requirements including grave plotting map creation, socket preparation with specific clearance dimensions (3" minimum), uniform headstone heights (25" above ground), alignment tolerances (1/8" maximum transverse/lateral, one degree vertical), and final cleaning. The third form addresses flat marker reset and realignment with seventeen requirements, including subgrade preparation (8-12 inches ripped depth with soil amendments), compaction standards (95% for subgrade and aggregate base, 90% for amended topsoil), plastic geogrid support system installation, and marker verification on correct gravesites. The fourth form covers turf renovation by sod installation, encompassing fourteen requirements including soil testing and recommendations, herbicide treatment protocols (minimum 10-14 days for translocation), power raking and debris removal, amended topsoil application (2 inches minimum with 4-inch tilling depth), compaction to 85% maximum dry density, irrigation system restoration, and Kentucky Bluegrass sod installation with specific joint and maintenance standards. Each form requires the Contracting Officer's Representative (COR) and contractor signatures, with space for detailed documentation of deficiencies and required corrective actions.

Upright Headstone Reset, Realign, Backfill & Clean Headstones STATUS
REQUIREMENT PER THE STATEMENT OF WORK Met *Did Not Meet
1. Contractor creates grave plotting maps that show specific locations of each headstone by actual number and name. Map is verified with COR. [ ] [ ]
2. Section keystones have been properly chosen with the approval of the COR in order to achieve a smooth, flowing transition between the rows and columns. [ ] [ ]
3. Headstones are removed from their sockets and placed on their gravesites. (Headstones are not to be removed from their respective gravesites.) [ ] [ ]
4.
Upright headstone sockets are dug with a 3" minimum clearance on all four sides and the bottom of the headstone to meet the height and alignment requirements. Sockets are clean and free of loose dirt and debris. The crushed stone aggregate has been added to the correct level and fully compacted at the bottom and along all sides of the headstone to the proper elevation leaving 4-inches for placement of topsoil.
5.
Upright headstones in burial sections are uniform in height (25") above ground (20" for older/smaller headstones), horizontally and vertically plumb, with inscriptions visible, and installed to ensure a pleasing top line while compensating for irregular terrain (sloping and uneven ground). Transverse and lateral alignment is within the maximum allowable tolerance of 1/8" and vertical alignment is within one degree of fully vertical when measured on front and sides using a digital level. COR has randomly sampled the headstones to verify height and alignment.
6.
Completed raised and realigned headstones in all soil and terrain conditions are firmly set in place so that the headstones are rigid with no give or play. COR has randomly checked to assure headstones are firmly set in place.
7. Headstones otherwise present a neat overall appearance in a line vertically, laterally & transversely and provide a uniformly flowing transition throughout the Section and with adjoining Sections. [ ] [ ]
8. Headstones have been cleaned in accordance with the contract requirements. [ ] [ ]
*Deficiency(s)): Detail below specific contract requirement(s)) which Contractor "Did Not Meet" and necessary corrective action. (Note: When deficiencies are cited, the Contractor is required to sign & acknowledge receipt of the report.)

Flat Marker Reset, Realign, Backfill & Clean Headstones STATUS
REQUIREMENT PER THE STATEMENT OF WORK Met *Did Not Meet
1. Contractor creates grave plotting maps that show specific locations of each marker by actual number and name. Map is verified with COR. [ ] [ ]
2. Section keystones have been properly chosen with the approval of the COR in order to achieve a smooth, flowing transition between the rows and columns. [ ] [ ]
3. Markers are removed from their sockets and placed on their gravesites. (Markers are not to be removed from their respective gravesites.) [ ] [ ]
4. Remove the existing plastic marker support system, leveling sand, and aggregate base. [ ] [ ]
5.
Subgrade is ripped to a depth of 8 to 12-inches; soil buster (gypsum equivalent) and compost are applied and tilled to a depth of 6-inches. Subgrade is leveled and compacted leaving depth for 2-inches of imported topsoil and soil amendments to establish finish grade.
6. A continuous trench is dug underneath the row of markers to accept the marker supports system, and the subgrade is compacted to 95% maximum dry density. [ ] [ ]
7. 3/4” minus crushed aggregate base is installed to design grade underneath the plastic support geogrid at marker locations only and compacted to 95% maximum dry density. [ ] [ ]
8. Amended topsoil is installed to design grade underneath the plastic support geogrid where sod is to be planted and compacted to 90% maximum dry density. [ ] [ ]
9. Plastic marker support geogrid is installed in continuous length for each row. [ ] [ ]
10. 1/4” minus crushed aggregate leveling sand is installed and compacted in the cells of the plastic marker support geogrid at marker locations only. [ ] [ ]
11.
Markers are correctly installed on plastic marker support geogrid: set in a line laterally, transversely, and diagonally with flat markers of other graves, and the top surfaces at proper heights and levels to provide a uniform flowing transition through the rolling terrain.
12.
Markers are verified to be reinstalled on the correct gravesites using temporary grave plotting maps, existing permanent control markers where available, and temporary contractor installed control markers precisely installed at the ends of each gravesite row
13.
Amended topsoil is placed and compacted in the cells of the plastic marker support geogrid.
Amended topsoil consisting of minimum of 2-inches of imported topsoil over 2-inches of existing topsoil is placed throughout the entire renovation area where sod is to be planted.
14. Imported topsoil and soil amendments are tilled to 4-inches depth into the native topsoil to create amended topsoil. [ ] [ ]
15.
Markers in all completed work areas are firmly set and anchored in place with no movement from forces subjected by the COR and correct in appearance, spacing, depth, alignment, plumbness, height, accuracy, and smoothness in grade transition.
16. Markers are cleaned using headstone cleaning agent and pressure washing. [ ] [ ]
17. Any contractor caused damage is repaired according to this Work Statement. [ ] [ ]
*Deficiency(s)): Detail below specific contract requirement(s)) which Contractor "Did Not Meet" and necessary corrective action.

Turf Renovation by Sod STATUS
REQUIREMENTS PER THE STATEMENT OF WORK: Met *Did Not Meet
(a) Soil samples collected and submitted to certified soil testing laboratory. Soil test report and recommendations furnished to the COR upon completion of the laboratory testing. [ ] [ ]
(b) Exposed components of the irrigation system, valve boxes, sprinklers, controllers, have been protected. [ ] [ ]
(c)
The entire vegetated area has been treated with an application of a non-selective and selective herbicides by a licensed pesticide applicator to kill weeds and existing turf. A minimum of 10 to 14 days has elapsed for the herbicides to fully translocate throughout all plant parts. The COR confirms that all vegetation in the treated area is completely dead.
(d) The entire treated area has been power raked or verti-cut and all residual plant debris including thatch has been removed. [ ] [ ]
(e) The area has been irrigated for a minimum of 14 days to encourage growth of any remaining plant material and germination of weed seed lying dormant in the topsoil. [ ] [ ]
(f)
The entire turf renovation area has been treated a second time with an application of a non-selective and selective herbicides by a licensed pesticide applicator to kill remaining, and newly germinated weeds and turfgrasses. A minimum of 10 to 14 days has elapsed for the herbicide to fully translocate throughout all plant parts. The COR confirms that all vegetation in the treated area is completely dead.
(g) Two inches (2”) of imported topsoil is applied. Fertilizer and soil amendments are broadcast onto the topsoil per soils report and thoroughly rototilled into the soil to a minimum depth of 4 inches to produce amended topsoil.
(h)
Amended topsoil has been graded, leveled, compacted to 85% maximum dry density, and raked to achieve a finished appearance that is smooth, uniformly level, and free of all surface ripples, depressions, high spots, low areas, ridges and stones larger than 1/2-inch. The finish grade is a maximum of the sod thickness below the adjoining grade of any finished surface and is nominally 25-inches below the tops of all upright headstones and 1-inch below the top of all flat markers unless otherwise approved by the COR.
(i) Irrigation equipment is re-installed, and irrigation system tested, repaired, and determined fully functional. [ ] [ ]
(j) Irrigation heads and valve boxes are brought to proper relationship with finish grade with tops of heads and valves boxes at 1-inch above finish grade. [ ] [ ]
(k) Turfgrass starter fertilizer has been uniformly broadcast onto the prepared sod bed with a properly calibrated fertilizer spreader at the rate specified for the specified Kentucky Bluegrass sod. [ ] [ ]
(l) Turfgrass sod has been installed using certified sod as specified. Sod pieces are not stretched or overlapped and that all joints are butted tightly to prevent voids that would cause air drying of the roots and to prevent weed growth, and lightly rolled to ensure proper soil contact.
(m) Turf is properly maintained during Turf Establishment Period as described in the Work Statement: weeds, diseases, pests eradicated, bare areas re-sodded, area correctly mowed, trimmed, and irrigated. [ ] [ ]
(n) Any contractor caused damage is repaired according to this Work Statement. [ ] [ ]
