About this file

This is a Quality Assurance Surveillance Plan (QASP) for the Tobyhanna Army Depot Grounds Maintenance Services contract, effective March 10, 2026 (Version 2.0). The QASP establishes procedures for the government to monitor contractor performance against standards outlined in the Performance Work Statement (PWS) and determine compliance with contract requirements.

The QASP identifies eight performance requirements with specific standards and inspection methods. High visibility area mowing must maintain grass between 2.5 and 3.5 inches (100% compliance via periodic inspection); weed removal from beds and hardscapes requires 100% compliance via periodic inspection; edge trimming along hardscapes requires 75% compliance via random sampling; snow shoveling/plowing response must occur within 2 hours of call-out with zero deviation via 100% inspection; ice pre-treatment of sidewalks with 100% brine requires 100% compliance via 100% inspection; quality control and reporting must be accurate, complete, and within suspense dates with zero deviation via 100% inspection; vegetative roof maintenance requires drains to be clear and plants maintained three times per season at 100% compliance via periodic inspection; and emergency tree removal must respond within 24 hours and take action within 48 hours with zero deviation via 100% inspection. The Contracting Officer is Stephanie Coleman (ACC-APG Tobyhanna Division), the Contracting Officer's Representative is Balut Matthew (Tobyhanna Army Depot), and contractor key personnel include the Project Manager and Quality Control Manager. Performance ratings range from Excellent (exceeds many requirements with few minor problems) to Unsatisfactory (fails to meet most requirements with unlikely timely recovery).

1. Vision

The vision of this Quality Assurance Surveillance Plan (QASP) is to provide the procedures necessary to monitor the Tobyhanna Army Depot Grounds Maintenance Services contract. This QASP offers the details and means to identify, record, and report deficiencies observed while comparing actual work throughout this service contract's period of performance against the standards outlined in the Performance Work Statement (PWS).

2. Mission

Tobyhanna Army Depot (TYAD) is a recognized leader in providing world-class logistics support for Command, Control, Communications, Computers, Cyber, Intelligence, Surveillance and Reconnaissance (C5ISR) Systems across the Department of Defense. To support these capabilities and its Warfighters, Tobyhanna Army Depot requires that its grounds, facilities, and access routes are maintained in a pristine, professional, and safe condition, ensuring uninterrupted operations regardless of weather conditions.

3. Purpose

This QASP is a government-developed document used to determine if the contractor's performance meets the performance standards contained in the contract. • What will be monitored

• How monitoring will take place

• Who will conduct the monitoring

• How monitoring efforts and results will be documented

The contractor is responsible for implementing and delivering performance that meets contract standards using its

Quality Control Plan (QCP). The QASP provides the structure for the government's surveillance to ensure it meets contract standards. The QASP is a living document and may be modified based on contractor performance.

The KO shall ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and safeguard the interests of the United States. The KO determines and documents the final assessment of the contractor's performance.

• Assigned KO: Stephanie Coleman

• Organization: ACC-APG Tobyhanna Division

5.2 Contracting Officer's Representative (COR)

The COR is responsible for providing continuous technical oversight of the contractor's performance. The COR uses the QASP to conduct the oversight/surveillance process and maintains a Quality Assurance file documenting actual performance.

• Organization: Tobyhanna Army Depot (TYAD)

5.3 Contractor Key Personnel
